1. Learn Oracle Questions, Questionnaire and


Questionnaire Template
In this article we will learn to create questionnaires. Questionnaire is a common
functionality and used across multiple modules but frequently used in oracle
performance management. I would also say that configuring questionnaires is
super easy, so without any further delay, let’s start it…

2. What are the different steps required to


configure questionnaire
Majorly there are three sections that we need to go through to complete our
questionnaire configurations, which are as follows

 Configure Questions : Here we create all questions that we need for questionnaire. A question can
be part of multiple questionnaire, which means questions can be reused.
 Configure Questionnaire Template : In template basically we design how these question would
appear on screen like vertically or horizontally, what would be the order of questions sequentially
or randomly etc. Same like questions questionnaire template can also be reused, so we do not need
to create multiple templates unless design changes.
 Configure Questionnaire : Questionnaire is nothing but set of questions grouped to serve some
specific purpose.
You can find link to configure them one after another under My Client Group
» Performance as shown below in the image. Will also tell what is the individual
task name for each one of them.
3. What are we going to configure today?
Before we start our actual configuration, let’s try to understand what we are going
to achieve.

 If you see the first column Question in the table below, those are the questions we will create.
 If you see the second column Questionnaire you will understand that we will be configuring two
questionnaires. First questionnaire Employee Questionnaire will contain first four questions and
second questionnaire Manager Questionnaire will contain remaining five questions.
 Third column Template tells that we will be configuring only one template and both these
questionnaire will use same template.

Could you please elaborate what improvements are required to deliver


a projects successfully?

Describe the workers contribution to the team and project?

How would you rate person’s performance in the project?

Select all the tasks that you think this person can assume for the  
coming year?

Would you recommend worker for promotion?


Manager
Please explain why do you think worker can be promoted? Questionnaire

Now we have got idea what we are going to configure let’s begin the
configuration.

4. Configure Questions for Questionnaire


To create questions search for the task Manage Question Library or navigate to
My Client Group » Performance » You will find link Manage Questions under
Setup Maintenance.
Before you actually start creating questions just want to tell that it is a good
practice to create separate folder for similar kind of questions or questions
created for specific requirement.

To create a new folder click your cursor over Question Library and then click
on + icon (Create). This will prompt you to enter folder name. Enter folder name
as AP Workers Evaluation Questions

Now click on new folder name you just created AP Workers Evaluation
Questions and then click on the + Create button available under Search
Results section.
You will be on Create Question page. Now enter details as follows
 Change the status from Draft to Active.
 Folder : AP Workers Evaluation Questions
 Privacy : Public
 Under the Details section you will see Question Text There you have to enter your actual question
text. Let’s enter our first question here that is “What was your greatest accomplishment in the
project?”
 Leave the Questions Type as Text.
 Under the Response Change the Response Type to Rich Text Box.
Leave everything else unchanged or accept default value.
If you click on the Question Type filed you will notice following values. Don’t
worry, we will create questions of all these types soon. We have just completed
one type that is Text.
 Text

 Single Choice

 Multiple Choice

 No Response
Once you enter details as mentioned above, click on the button Save and Close.
You will get confirmation message, click on OK to acknowledge it.
Now repeat these steps to create following three questions, which are of same
questions type Text
 Could you please elucidate where you could not perform as per the expectation?

 Could you please elaborate what improvements are required to deliver a projects successfully?

 Describe the workers contribution to the team and project?

Total we have created four questions so far. All these questions had Question
Type as Text. Now we will create two questions of Question Type as No Response.
Once again click on the + Create button while folder AP Workers Evaluation
Questionnaire is still selected and enter details as follows.
 Change the status from Draft to Active.
 Folder : AP Workers Evaluation Questions
 Privacy : Public
 Question Text: If there was any achievement or appreciation email you got, please attach evidence
to support it.
 Question Type : No Response
 Allow Attachments : Yes
In the No Response type question there would be no filed to enter text, since we
have enabled attachment, only attachments can be added as response.
Once you enter details as mentioned above, click on the button Save and Close.
You will get confirmation message, click on OK to acknowledge it.
Now our next question is of type Single Choice, to create it click on the +
Create button while folder AP Workers Evaluation Questionnaire is still selected
and enter details as follows.
 Change the status from Draft to Active.
 Folder : AP Workers Evaluation Questions
 Privacy : Public
 Question Text: Would you recommend worker for promotion?
 Question Type : Single Choice
 Response Type : Radio Button List
Click on the + Add button and enter detail as follows in the response area.

Response Code Short Description

<Leave System Default> Yes

<Leave System Default> No


Once you enter details as mentioned above, click on the button Save and Close.
You will get confirmation message, click on OK to acknowledge it.
Next, we will create multiple choice question, click on + Create button while
folder AP Workers Evaluation Questionnaire is still selected and enter details as
follows.
 Change the status from Draft to Active.
 Folder : AP Workers Evaluation Questions
 Privacy : Public
 Question Text: Select all the tasks that you think this person can assume for the coming year?
 Question Type : Multiple Choice
 Minimum Number of Selections : 2
 Maximum Number of Selection : 3
 Response Type : Check Multiple Choice
Click on the + Add button and enter detail as follows in the response area.

Response Code Short Description

<Leave System Default> Mentoring

<Leave System Default> Coaching

<Leave System Default> Teaching

<Leave System Default> Project Leading

<Leave System Default> Resource Managing


Once you enter details as mentioned above, click on the button Save and Close.
You will get confirmation message, click on OK to acknowledge it.
In the next question we will demonstrate how you can your use any rating model
of value set

To create the next question click on + Create button while folder AP Workers
Evaluation Questionnaire is still selected and enter details as follows.
 Change the status from Draft to Active.
 Folder : AP Workers Evaluation Questions
 Privacy : Public
 Question Text: How would you rate person’s performance in the project?
 Question Type : Single Choice
 Response Type : Single Choice from List
 Rating Model : AP Performance Rating Model

Model
Once you select rating model, you will notice that values from that rating model
appear automatically as shown below in the image.
Once you enter details as mentioned above, click on the button Save and Close.
You will get confirmation message, click on OK to acknowledge it.
Now we will create last question, which is a bit tricky. This question should
appear only if manager recommends promotion. To see how it is done click on +
Create button while folder AP Workers Evaluation Questionnaire is still selected
and enter details as follows.
 Change the status from Draft to Active.
 Folder : AP Workers Evaluation Questions
 Privacy : Public
 Question Text: Please explain why do you think worker can be promoted
 Question Type : Text
 Display the question conditionally : Yes
 Controlling Question Code: Click on the search link and search for question “Would you
recommend worker for promotion?” and select it.
 Controlling Response : Yes
 Response Type : Rich Text Box
 Allow Attachments : Yes
 Minimum Length : 100
 Maximum Length : 2000

Once you enter details as mentioned above, click on the button Save and Close.
You will get confirmation message, click on OK to acknowledge it.
Now we have created all our questions. Total nine questions. The next step is to
create Questionnaire Template. Let’s do it too.

5. Configure questionnaire Template


To configure questionnaire template search for the task Manage Questionnaire
Templates or navigate to My Client Group » Performance » Manage
Questionnaire Templates
Our objective here is not to create any fancy template but a general purpose
template that can be used by various questionnaires without any issue.

When you are on Manage Questionnaire Template page click on + Create


button and you will on page “Create Questionnaire Template: Basic Information”.
On this screen enter details as follows
 Subscriber : Performance 360
 Name : AP Questionnaire Template
 Status : Active
 Privacy : Public
 Score Questionnaire : No
 Owner : Leave default value
 Allow changes to instruction : Yes
 Description : <Description of your choice or leave blank>
 Allow changes to instruction : Yes

Click on Next button and you will be on “Create Questionnaire Template:


Contents”. Nothing much is needed to change here just ensure you have entries
like this
 Section Order: Sequential
 Section Presentation : Stack Regions
 Page Layout : 1 Column
 Allow changes to format options : Yes
 Allow changes to sections : Yes
 Allow Responses Type : All
Now under Sections heading you might see two rows. Best thing to move forward
is delete both of them and make fresh entry by clicking on + Create button and
enter details as follows (or you can just delete one of them and rename other one
as follow)
 Title : Questions
 Allow Additional Questions : Yes
 Question Order : Verticle
 Response Order : Verticle
 Required : No

Click on Next button and you will be on Review Page. Review your setup and if
everything looks okay, click on Save and Close button. Acknowledge the
confirmation by clicking on OK button.
6. Configure Questionnaire
So far we have created questions and Questionnaire Template now the time is to
create Questionnaire, which will make use of our earlier configurations. To create
questionnaire search for the task Manage Questionnaires or navigate to My Client
Group à Performance à Manage Questionnaires
6.1. Configure Questionnaire for Employees
On your left hand side you will see Questionnaire Library select that and then
click on + button (create) to create a questionnaire. Enter name as “AP Worker
Questionnaire”
Now select AP Worker Questionnaire folder and click on + Create button as
shown below.

As soon as you click on + Create button, Create Questionnaire pop up window


will appear. There you have to search questionnaire template we created earlier.
Remember the funda(logic), Questionnaire usages Questionnaire Template to
display Questions.  Just search for the template we created earlier “AP
Questionnaire Template” and click on OK.
You will be on “Create Questionnaire: Basic Information” page. On this screen
enter details as follows

 Name : AP Employee Questionnaire


 Status : Active
 Folder : AP Worker Questionnaire
 Privacy : Public
 Owner : Your Name or leave it default
 Description : Anything you want
 Instruction and Help Material: Any instruction that should be visible to employee. Something
like “Please go through each question and give your honest reply without any hesitation.”
Click on Next button. You are on Create Questionnaire: Contents page. This
screen looks more like Questionnaire Template we configured earlier, right? Yes,
that is because we are using that template. We will leave everything defaulted,
just scroll below at Questions: Questions section.
Here you can add questions that we configured earlier. Remember
this Questionnaire is only for employees, so we will be adding questions only
related to employees.
Click on the + Add button, which will ask you to search question that you want to
add. Search for first question “What was your greatest accomplishment in the
project?” and then click on OK.

Now repeat the steps to add all four questions related to employees, which are
as follows

If there was any achievement or appreciation email you got, please Employee  
attach evidence to support it. Questionnaire
Once done, you will see all your four questions added there, If you see the
required column, none of them are required at the moment, lets mark the first
question required so that employee has to compulsorily answer that question.

Once done, click on Next and you will be on “Create Questionnaire: Review”


page. Click on the Preview button to see how questions would appear for
employees. Once satisfied click on Save and Close button. You will get
confirmation message, just click on OK button.
6.2. Configure Questionnaire for Managers
We need to configure one more questionnaire for Managers and add questions
pertaining to managers. I would not be going through steps in details, you may
have got idea from employee questionnaire, so just follow same steps for
Manager Questionnaire. You will be using same Questionnaire Template.

We will be adding following questions to Manager Questionnaire. Keep the first


four questions mandatory.
