St. Mary’s Academy of Sto. Niño collects and uses personal information (referred to in the R.A.
1073 Data Privacy Act of 2012 as personal information) about staff, pupils, parents, and other
stakeholders who come in contact with the school. We ensure the safety of your personal
information through the school’s data privacy policy. All information collected will be used for the
provision of education and other associated functions.

In line with this, we are asking the consent of our dear parents for the said collection and
utilization of all personal information gathered by the school. Consent forms will be distributed
and retrieved per section through the advisers. Attached below is the soft copy of the consent
form.

Parents/Guardians are asked to:


1. Read and understand the contents of the consent form.
2. Fill in and sign the consent form.
3. Return the signed consent form.

St. Mary’s Academy of Sto. Niño is one with you in delivering quality distance and online
education this school year.

I understand that these images may be posted in the handbook to give students and parents
and/or guardians an understanding of the general rules and guidelines of the school specifically
in wearing of the school’s prescribed uniform. I am fully aware that these images and/or videos
can be posted to the school’s FB page, Help Desk, and website if and when needed during
classes and activities of Student Support Services using the Learning Management System (MS
Teams and Kleafs) for us to see the different activities provided by the school for School Year
2022-2023.
Since anyone can download the images and/or videos from the internet or reproduce copies of
these, I agree that St/ Mary\s Academy of Sto. Niño will not be liable or responsible for its
unauthorized use.

I am aware that I am not entitled to any compensation out of the use of such images and/or
videos.

By affixing my signature below is an acknowledgement that the school has answered all my
queries and I fully understand this agreement.
