ACCESSORS SCRIPT
Before THE CONDUCT OF THIS ASSESSMENT I ensure the facilities, tools and equipment
are ready and available (WORKSHOP INSPECTION CHECKLIST)
A. INTRODUCTION
1. Introduction of Accessor
 Good morning everyone! I am Ms. Angelyn R. Tansiongco Your competency Accessor in
Organic Agriculture Production NCII
2. Acknowledge the presence of the processing officer, AC manager and the TESDA
representative
 I would like to acknowledge the presence of Ms. _______ our processing officer who
prepares the documents needed for the assessment. Mr. ______ our AC Manager, whom
the processing officer submits the documents and Mr. _______ our TESDA representative.
Good morning Ma’am, good morning Sir!
 You do not have to worry about their presence they are just here to oversee the conduct of
assessment and to check if the conduct of assessment is in accordance with the
prescribed competency standards set by TESDA.
3. Prayer
 Before we formally start our assessment, may I request Ms. ________ to lead us a short
prayer. Thank you.
4. Collect the admission slips
 Before we start please submit your admission slip
5. Instruct candidates to sign the attendance sheet
 For your attendance please sign the attendance sheet
6. Present accomplished application form with self-assessment guide and explain the
significance.
 I have here your accomplished application form and self-assessment guide. This is an
indication that you are knowledgeable and can perform all the units of competency
enumerated in Organic Agriculture Production NC II
7. CCTV recording for validation
 This assessment will be CCTV recorded for the validation and integrity of this
assessment

B. CONDUCT ORIENTATION TO THE CANDIDIDATES
1. Explain the context and purpose of the assessment
 I would like to welcome all of you to our center, Romblon Institute for your assessment in
Organic Agriculture Production NC II. You are very blessed because this assessment
center was also your training center. In that case, you are already familiar with the area
as well as the tools that we will be using later.
 The purpose of our assessment is to determine the level of your competence.
2. Qualification of units of competency to be assessed
 The Organic Agriculture Production NC II qualification is composed of basic, common
and core competencies.
 Core competencies consists of the following:
1. Raise Organic Chicken
2. Raise Organic Vegetable
3. Produce Organic Fertilizer
4. Produce Various Concoctions

On elective competencies you can choose between Raise Organic Hog and Raise
Organic Small Ruminants
3. Task to be performed and evidences to be collected
 For your assessment you will perform the production organic concoction which is the
Fish Amino Acid
You will be group by 5 and will be given 30 minutes to process the concoction that will
be assigned to your group.
4. Assessment procedures to be undertaken
 To determine your knowledge and skills in Organic Agriculture Production NC II we
will collect evidences through demonstration with oral questioning and written test.
This assessment method can help measure your underpinning knowledge and skills.
5. Needs of the candidates to be considered during the assessment
 For your personal needs you can take your meal in our canteen over there. The lavatory is
located beside our exit on the left side. Just ask permission form me since we are
conducting our assessment. However, during the assessment you are not allowed to leave
the premises of the assessment center
6. Adjustments when applicable
 If you have difficulty understanding the given instructions, please do not hesitate to
inform me because I am very much willing to translate it for you. Also during the written
examination if you have dilemma writing down your answer don’t be shy because we can
accept your answers verbally. Tagalog dialect is very much acceptable.
7. Do’s and don’ts during assessment
 If there are things you want to clarify regarding the assessment, you may raise your right
hand. I am more than willing to assist you, but once the assessment has started please
refer from asking questions for it is no longer allowed. If you have physical or any form of
difficulties do not hesitate to call my attention so that we can address it immediately.
 Kindly put your phones on silent mode for us to concentrate on the conduct of
assessment.
8. Rights and appeal system
 In case you have questions regarding with your assessment results, you have the right to
appeal. You can submit your appeal to the TESDA provincial office for the assessment
center and the assessor.
9. Issuance of national or certificate of competency
 For the issuance of the National Certificate the AC manager shall inform you when you
can claim your National Certificate after 5 working days after the assessment.
10. Reassessment procedures
 After the assessment and you found not to be competent, don’t worry about it. You can be
reassessed anytime at your own convenience. The reassessment shall focus only in the
unit of competencies you found not yet competent. But, if you failed two consecutive
assessments you will be advised to undergo a refresher course before applying a
reassessment.
11. Occupational Safety and Health Requirements
 During the actual assessment please observe safety as you perform the different task. In
case of accidents please call my attention immediately so that we can provide the first aid.
Our first aid is located on the wall of the _______ area.
12. Confidentiality of Assessment Outcomes
 With regards to the results of your assessment we observe strict confidentiality; this is to
ensure our integrity. The results will only be available to authorized personnel and to the
institution where you had the actual training. In relation to this I ask all candidates to

refrain from giving away any form of information that took place during the assessment?
Do you have any question or clarification?
13. Provide each Candidate a Copy of the Specific Instruction
 If you don’t have any question or clarification, then I am going to distribute the specific
instruction, please read carefully. If you need assistance do not hesitate to ask me, I am
more than willing to assist you.
14. Assign numbers and group to the candidate
 Now, I will give you an assigned number and group
 Since everyone have their respective group and numbers you can now have your 15
minutes break.
 After your break please proceed to the ASSESSMENT AREA for your written test,
understood? (Yes, Ma’am)
C. COLLECT THE EVIDENCE AND MAKE THE ASSESSMNET DECISION
1. To stimulate the assessment process, two candidates will be shown performing the
tasks.
2. All candidates will represent being competent where the others is not yet
competent
3. Explain to the candidate the task to be performed
15. Administer written test (all the students)
 Since we are done with our orientation is there any question or clarifications? (None
Ma’am) okay, if none then we will proceed to your written test. Please follow me to the
ASSESSMENT AREA
 Here is your written test, please pass
 Read the directions carefully and answer all the questions. I will give you 1 hour to
answer. Time’s up, please pass. Thank you
16. Demonstration (RATING SHEET)
 Since we are done with the written test let’s proceed with your demonstration.
 Group 1 please remain for your demonstration and the rest of the candidates please
proceed to the WAITING AREA.
 Okay, you will now demonstrate how to process the Fish Amino Acid
 I HAVE HERE THE RATING SHEET
 Using the materials, tools and equipment, you are required to perform the tasks within
1 hour. I will be observing you while you are performing the tasks.
After your demonstration, I will ask you some questions related to your demonstration.
 You may call me when there is a need for me to translate or explain items for clarification.
 After the questioning portion, I will give you feedback about your performance.
 Any questions/clarifications? If there are no more questions, you may start now.
 Please proceed to the waiting area
 Please call the next group
 Please call candidate number 1
17. Oral questioning
 Please have a seat candidate number 1. Are you ready? (Yes Ma’am) Okay.
 Prepare questions
 Thank you. You may now proceed to the waiting area and please call candidate number 2.
I’ll call you back later to give the feedback regarding your assessment.

D. PROVIDE FEEDBACK ON THE ASSESSMENT


 The assessor must provide candidate about the outcomes of the assessment
process
 For positive feedback, COMPETENT

How do you feel about the assessment?
“Ma’am I am confident with my performance because I was able to perform well
the demonstration. Even if I had a hard time at written test and oral questioning”.
 My observation co-inside your opinion. You are competent in performing the different
skills in Organic Agriculture Production.
 However I have noticed that you did not observe proper body mechanics in processing
the organic concoction. Furthermore you did not confidently answer my question
during oral questioning.
 Nevertheless, you were able to answer all questions correctly and the result of the
written test shows that you are knowledgeable enough in Organic Agriculture
Production. In this manner, I would like to congratulate you for a job well done and
found that you are competent. Please sign the COMPETENCY ASSESSMENT RESULT
SUMMARY.
 For the issuance of the national certificate the AC manager shall inform you where to
claim the national certificate after 5 working days. Congratulations! And please do not
lose COMPETENCY ASSESSMENT RESULT SUMMARY (CARS) because you will
present it to the TESDA office in claiming the national Certificate. Please bring 50
pesos and the standard Passport picture for your certificate. Again, congratulation!

 Candidate number 2 please have a seat. Ms. Rapada
 How do you feel about the assessment?
“Ma’am I am confident with my performance because I was able to perform well
the demonstration. Even if I had a hard time at written test and oral questioning”.
 Okay. Actually you are very good at processing the Fish Amino Acid, however you did
not answer the oral question confidently on other unit of competency. Based on the
result of your written test and interview the outcome of the assessment is that you’re
not yet competent. Don’t worry you can still be re-assessed anytime at your own
convenience.

 If you have doubts regarding the result of the assessment you have the right to appeal
to the TESDA Provincial Official where the accessor and the assessment center are
accredited.
 And please sign the COMPETENCY ASSESSMENT RESULT SUMMARY and the
RATING SHEET, thank you. Please do not lose your CARS because you will present it
to the accessor when you are ready to undergo the re-assessment. Thank you and God
bless.
After the assessment I will record the outcome of the assessment and make sure that the
rating sheet are signed by the candidate. After this the assessment package will be given to

the AC Manager. And the AC manager shall give the assessment package to the processing officer.
Then the processing officer will accomplish the NC
