ELECTROCARDIOGRAPHY (ECG)

1. DEFINITION
ECG electrocardiogram (ECG) is a graph created by a lethrocardiograph, which
records the electrical activity of the heart in a certain time. An electrocardiogram or
what we usually call an ECG is a record of the electrical activity of the heart caused
by the special excitation and conductive system of the heart. A normal heart has
impulses that arise from the SA node then are delivered to the AV node and purkinje
fibers. It is this impulse journey that will be recorded by the ECG as a tool for
analyzing the heart's electricity. According to Heru (2018), ECG is a graphic picture
of the peak picture of electrical activity from the heart muscle fibers, in the form of a
time function voltage curve consisting of various peaks. An ECG can be used to
measure heart rate and diagnose any abnormalities in the heart.

3. Electrodes for precordial and extremities
4. Skintact ECG
5. ECG paper, horizontal and vertical line graphs with a distance of 1 mm. Thicker
lines are found every 5 mm.

4. PROCEDURES
1. Explain the purpose of the action and procedure of the ECG examination to be
carried out.
2. Then ask the patient's readiness to do ECG recording
3. Then install a sampiran to maintain the patient's privacy. Next the device is
brought closer to the patient,
4. This procedure lasts approximately 10-15 minutes
5. When using jewelry / metal / gadgets so that they are removed and placed not
close / attached to the patient
6. After everything is ready, I wash my hands 6 steps, put on handscoons and
immediately take action according to the SOP
7. The patient is asked to undress the chest.
8. The patient is welcome to sleep on his back, the examiner's position is to the right
of the patient.Pasien diusahakan untuk tenang dan bernafas normal. Selama proses
perekaman tidak boleh bicara.
9. Clean the area to which the electrodes will be installed with an alcoholic cotton
swab.
10. Perform the installation of an ECG skintact by attaching it to the extremities and
precordial part of the patient.
11. do the insertion in the extremities first and then in the precordial, the reason is so
that the chest area is not exposed for too long (maintaining the patient's privacy)
12. Installation location
Installing extremity electrodes :
Red (RA) : right wrist in the direction of the palm
Yellow (LA) : left wrist in the direction of the palm
Green (LL) : inner left footBlack
Black(RL) : inner right leg
Installing pre cordial electrodes
V1 : 4th intercosta (ICS) on the dextra sternal line
V2 : 4th intercosta (ICS) on the sinstra sternal line
V3 : located between V2 and V4
V4 : Intercosta (ICS) to 5 on the mid clavicle line of sinistra
V5 : parallel to C4 on the anterior axillary line
V6 : parallel to C4 at the midaxillary line
13. Record ECG by pressing the ON then STAR buttons, then when finished press
OFF
14. Remove the electrodes then clean the jelly with tissue
15. Re-tidy up the device and position the patient as comfortable as possible
16. Termination: validate the patient's feelings after ecg recording
DS: Cooperative patient during ECG recording
DO: The patient appears to be following the directions given

18. Record on the upper left side of the ECG paper: Patient name, No.CM, date and
time of recording
5. RESULTS
The result of the patient's ECG interpretation with sinus rhythm
6. HAL-HAL YANG HARUS DIPERHATIKAN
Based on the journal (Adam, 2017) things that need to be considered in installing an
ECG are:
1. During installation the patient is freed from metal materials such as: watches,
waist straps, glasses.
2. Patients are recommended not to do activities, talk and move because it will
affect the results of the ECG
3. The patient's efforts do not touch the metal, the feet do not touch the bed and
the hands do not touch the bed
