Introduction and Purpose


Polarcus conducts offshore seismic exploration surveys worldwide and has developed this
Project EHSQ Plan to ensure that all operational Safety, Health, Security and Environmental
considerations are considered and effectively managed for the duration of this survey.

The main purpose of the Project EHSQ Plan is to:

 Provide assurance of the effective working Interface between the HSE management
system of Client, Contractor and Suppliers at the Project level, and to document this
Interfacing. It also includes the necessary Interfacing between other parties that may be
impacted by the Project. For example, local communities, production centers, offshore
platforms.
 Demonstrate that all involved parties have the necessary procedures (Permit to Work
(PTW); Hazard and Risk controls; Operating instructions; Lock Out/Tag Out (LOTO);
Manual of Permitted Operations (MOPO); Management of Change (MoC); Emergency
Plans) and controls in place to achieve the Project program without compromising EHSQ
performance. These systems shall be harmonized where possible to minimize the
potential for misunderstanding.

2. Description of the Project


The project is in Brazilian waters approximately~100 nautical miles East of Macaé, State of Rio
de Janeiro in the Albacora, Marlim and Voador concessions, Campos Basin.

Polarcus Naila will be utilized as the streamer acquisition vessel for this program which is
planned to commence on or about 08th July 2020 for approximately 300 days inclusive of
mobilization, re-configurations, and demobilization time. The Polarcus Naila will be
accompanied by 1 Support Vessel (SSV Venture- G) and 2 Guard / Chase (MV Ocean Dream &
MV Ocean Route ) vessel.
3. Environmental Impact Assessment - RIAS
Environmental Impact Assessment is outstanding. In order to avoid potential effects on marine
mammals, adherence to the mitigation measures set out in “Guidelines for Monitoring Marine
Biota in Maritime Seismic (2018) from IBAMA” will be undertaken.

The Lead MMO / onboard will also make sure everyone is aware of the mitigation measures to
be put into place. They will be responsible for verifying that the prevention and mitigation
measures required by “Guidelines for Monitoring Marine Biota in Maritime Seismic (2018) from
IBAMA” are incorporated into the Contractor’s final Risk Assessment. The Lead MMO will
prepare a Prevention and Mitigation Measures Checklist for use during the Survey.
4. Project Operations
4.1 Acquisition Plan Summary
The survey will be acquired in a “racetrack” mode but with 4D Repeatability considerations with
program line azimuths running 24.9° / 204.9°as seen on the map below.

A 24 hour look ahead notifications will be provided to all the stakeholders as required.
5. Management System Interfaces
5.1 EHSQ Reporting
All EHSQ events are reported and recorded within “Insite” within Polarcus and associated to
[Title] support/chase vessels.

This is the company wide electronic EHSQ logging software freely available across all PC’s on
board all Polarcus vessels and shore side operations. This software however cannot be made
readily available to the supply or chase vessels working with the acquisition vessel.

Such vessels will continue to use their own EHSQ reporting materials and systems as mandated
by the relevant Bridging documentation, articles, and agreements between them and Polarcus.
Such EHSQ reports will be transferred electronically daily to a designated Polarcus seismic
vessel. All EHSQ records and reports shall be made available throughout the project to the on-
board Client including both Lead QC, via their own dedicated Insite access.

6. Risk Management
The Management of Change (MoC) provides a structured 4-step approach to transitioning from a
current state to a desired future state. It can be any planned or enforced change, temporary or
permanent to plant, technology, process, products, services, compliance requirements,
procedures, people, or organization. The 4 steps outlined in the Management of Change
Procedure are:

 Step 1 – Identify
 Step 2 – Risk Assess
 Step 3 – Implement
 Step 4 – Complete

The MoC process is delivered through Polarcus reporting system Insite which can be accessed by
client representatives onboard.
