chairs and other resources to be used in the implementation of SHS program
___ 6. Inventory of learning resources prepared by school’s Property Custodian, as validated by the
Schools Division Office
___ 7. Updated Personal Services Itemization and Plantilla of Personnel of the concerned school
___ 8. Updated Status Report with regard to the school’s existing crucial resources
___ 9. Map, preferably drawn to scale, showing the vacant lot where the proposed SHS classroom/school
buildings are/will be constructed, duly certified by the City/Municipal Engineer
___ 10. List of prospective enrollees in the SHS per track and strand, indicating their names, Learner
Reference Numbers (LRNs), where applicable, ages, addresses, school names and DepED school ID
Numbers where they are currently or previously enrolled
___a. Justification signed by the SDS, in case the required minimum enrolment and/or number of
tracks are not satisfied
___ 11. List and types of establishment and industries in the community, as attested to the Department of
Trade and Industry, Department of Labor and Employment or the Municipal Planning Officer
___ 12. Certification from the SDS that the track(s) and strand(s) to be offered are aligned with the Local
Development Plans, as evident in the list provided by the City/Municipal Mayor, and are decided
upon by the Regional Director, SDS, Division Planning Officer and the School Head concerned

Madam:

This is to express our intent to change our specialization from General Academic
Strand (GAS) to Humanities and Social Sciences (HUMSS) under the Academic Track starting
SY 2022-2023 for the following reasons:
1. Since GAS does not offer a specialized course, it will be for the advantage of our
learners who are contemplating to go to college after high school, if they take up
HUMSS which is a specialized strand. With this strand offering a specialization, we
will be helping our learners to gain better preparation for higher education
particularly in selecting a particular career path.
2. Majority of our incoming Grade 11 learners are choosing HUMSS as their number 1
preference in the Senior High School Academic Track.
3. Shifting from GAS to HUMSS will not necessarily entail shortage in items of our
crucial resources, like teachers, classrooms, and armchairs. The school has capable
incumbent teachers who can efficiently handle the specialized subjects in HUMSS.
The existing classrooms and armchairs for Grade 11-GAS are similarly adequate
even if the said change will be implemented.
In view of the preceding explanations, we look forward to your favorable action
on the herein request.

MEMORANDUM OF AGREEMENT

KNOWN ALL MEN BY THESE PRESENTS:

This agreement made and entered into by and between:

The ANDA INTEGRATED SCHOOL a duly registered and recognized EDUCATIONAL


INSTITUTION, with postal address at POBLACION, ANDA, PANGASINAN represented by its
Principal Carla Fatima D. Soriano hereinafter referred to as “EDUCATIONAL INSTITUTION”.

The CHICKTASTIC BISTRO PANGASINAN a Private Business existing under the Law of the
Republic of the Philippines, with the address at BRGY. POBLACION represented herein by its
Manager JERALD CUBOS and hereinafter referred to as “INDUSTRY”.

WITNESSETH, that
WHEREAS, the educational institution is offering courses in SENIOR HIGH SCHOOL and has
instituted and IMMERSION PROGRAM wherein qualified students will undergo training prior to
graduation in order to be adequately familiar with actual work/job atmosphere to augment
their formal training.

WHEREAS, the EDUCATIONAL INSTITUTION proposes that its students be allowed to undergo
IMMERSION Training Program at the “INDUSTRY”, which proposal was accepted/approved by
the latter, subject to terms and conditions provided for in this Agreement (the On-the-Job
Training Program).

WHEREAS, the “INDUSTRY” agreed to accept students of the “EDUCATIONAL INSTITUTION” as


trainees at its various facilities in support of the EDUCATIONAL INSTITUTION’s IMMERSION
Training Program and complement existing to match demand;

NOW, THEREFORE, in consideration of the “EDUCATIONAL INSTITUTION” and the “INDUSTRY”


agrees and stipulates the following:

A. THE EDUCATIONAL SHALL:

1. Designate a Program Coordinator who will supervise the On-the –Job Training Program and
coordinate with the “INDUSTRY” various activities under the On-the-Job Training Program;

2. Pre-select and recommend qualified students who will undergo the training taking into
Consideration the requirement of the “INDUSTRY in terms of qualification and number of
trainees needed, it being understood that only students taking courses wherein training is an
academic requirement can be recommended;

3. Submit the “INDUSTRY” the resume and all other documents required under the On-the Job
Training Program, which shall serve as the basis of the “INDUSTRY” to screen, select and deploy
specific students for the On-the-Job Training Program;
4. Orient students about rules, proper decorum, behavior and work ethics inside the
“INDUSTRY” premises;

5. Monitor attendance, activities and performance of students during the On-the-Job Training
Program
6. Monitor compatibility of job circumstances with the course requirements for the students’
attainment of advanced standing in the program through regular interaction with the student-
training and regular visit to the training site;

7. Together with the students and parents concerned, renounce and waive any claim against the
“INDUSTRY” for injury that the students may sustain or loss that they may suffer, in any form,
whether personal and pecuniary arising from the fault or negligence of the students in the
performance of their duties and functions while undergoing the IMMERSION PROGRAM. The
waiver of the student trainees and their respective parents (as may be necessary) shall be
submitted to the “INDUSTRY” prior to the commencement of the IMMERSION Training Program.

This Agreement shall take effect immediately upon signing hereof and shall continue for a
period of 4 years thereafter. Unless otherwise advised in writing either party, this Agreement
shall automatically renewable for the same period under the same terms and conditions.
Notwithstanding the foregoing, the “INDUSTRY” or the EDUCATIONAL INSTITUTION, reserves
the right to terminate this AGREEMENT, subjected to fifteen 9150 day prior written notice to
the other party.

C. MISCELLANEOUS

This agreement shall be governed by and construed in accordance with the law
of the Republic of the Philippines.
A party shall not assign any of its rights and obligations under this Agreement,
except that the “INDUSTRY” reserves the right to assign student-trainee to any of its
subsidiaries and affiliates if such assignment is deemed by the “INDUSTRY” as necessary or for
the best interest of the student-trainee.
IN WITNESS WHEREOF, the parties hereof have signed this Agreement on this _____ of

This agreement made and entered into by and between:

The ANDA INTEGRATED SCHOOL a duly registered and recognized EDUCATIONAL


INSTITUTION, with postal address at POBLACION, ANDA, PANGASINAN represented by its
Principal Carla Fatima D. Soriano hereinafter referred to as “EDUCATIONAL INSTITUTION”.

The ANNA’S KAINANG PINOY a Private Business existing under the Law of the Republic of the
Philippines, with the address at BRGY. POBLACION represented herein by its Manager ANALIZA
CATABAY and hereinafter referred to as “INDUSTRY”.

WITNESSETH, that
WHEREAS, the educational institution is offering courses in SENIOR HIGH SCHOOL and has
instituted and IMMERSION PROGRAM wherein qualified students will undergo training prior to
graduation in order to be adequately familiar with actual work/job atmosphere to augment
their formal training.

WHEREAS, the EDUCATIONAL INSTITUTION proposes that its students be allowed to undergo
IMMERSION Training Program at the “INDUSTRY”, which proposal was accepted/approved by
the latter, subject to terms and conditions provided for in this Agreement (the On-the-Job
Training Program).

WHEREAS, the “INDUSTRY” agreed to accept students of the “EDUCATIONAL INSTITUTION” as


trainees at its various facilities in support of the EDUCATIONAL INSTITUTION’s IMMERSION
Training Program and complement existing to match demand;

NOW, THEREFORE, in consideration of the “EDUCATIONAL INSTITUTION” and the “INDUSTRY”


agrees and stipulates the following:

A. THE EDUCATIONAL SHALL:

1. Designate a Program Coordinator who will supervise the On-the –Job Training Program and
coordinate with the “INDUSTRY” various activities under the On-the-Job Training Program;

2. Pre-select and recommend qualified students who will undergo the training taking into
Consideration the requirement of the “INDUSTRY in terms of qualification and number of
trainees needed, it being understood that only students taking courses wherein training is an
academic requirement can be recommended;

3. Submit the “INDUSTRY” the resume and all other documents required under the On-the Job
Training Program, which shall serve as the basis of the “INDUSTRY” to screen, select and deploy
specific students for the On-the-Job Training Program;

4. Orient students about rules, proper decorum, behavior and work ethics inside the
“INDUSTRY” premises;
5. Monitor attendance, activities and performance of students during the On-the-Job Training
Program

6. Monitor compatibility of job circumstances with the course requirements for the students’
attainment of advanced standing in the program through regular interaction with the student-
training and regular visit to the training site;

7. Together with the students and parents concerned, renounce and waive any claim against the
“INDUSTRY” for injury that the students may sustain or loss that they may suffer, in any form,
whether personal and pecuniary arising from the fault or negligence of the students in the
performance of their duties and functions while undergoing the IMMERSION PROGRAM. The
waiver of the student trainees and their respective parents (as may be necessary) shall be
submitted to the “INDUSTRY” prior to the commencement of the IMMERSION Training Program.

This Agreement shall take effect immediately upon signing hereof and shall continue for a
period of 4 years thereafter. Unless otherwise advised in writing either party, this Agreement
shall automatically renewable for the same period under the same terms and conditions.
Notwithstanding the foregoing, the “INDUSTRY” or the EDUCATIONAL INSTITUTION, reserves
the right to terminate this AGREEMENT, subjected to fifteen 9150 day prior written notice to
the other party.

A. MISCELLANEOUS

This agreement shall be governed by and construed in accordance with the law
of the Republic of the Philippines.
A party shall not assign any of its rights and obligations under this Agreement,
except that the “INDUSTRY” reserves the right to assign student-trainee to any of its
subsidiaries and affiliates if such assignment is deemed by the “INDUSTRY” as necessary or for
the best interest of the student-trainee.
IN WITNESS WHEREOF, the parties hereof have signed this Agreement on this _____ of
