Research Project Title: Current Discipline Issues Among the Students.

The interview will last around an hour. Although we do not expect any hazards as a
result of your involvement, you have the right to end the interview or withdraw from
the study at any time.

Thank you for consenting to be interviewed for the study project mentioned above.
Interviewers must expressly consent to being interviewed and how the material
gathered during the interview will be utilized, according to SLA standards for
academic research conducted by Management & Science University (MSU)
institutions. We need this permission form to make sure you understand the goal of
your participation and that you agree to the terms of your participation. Would you
please read the information sheet that comes with it and then sign this form to indicate
that you agree to follow it?

• The interview will be recorded and a transcript will be produced.


• You will be sent the transcript and given the opportunity to correct any factual
errors.
• The transcript of the interview will be analyzed by our group member as a
research investigator.
• Access to the interview transcript will be limited to our group members and
academic colleagues and researchers with whom he might collaborate as part
of the research process.
• The actual recording will be kept or destroyed, stating what will happen.
• Any summary interview content or direct questions from the interview made
available through academic publication or other academic outlets will be
anonymized so that you cannot be identified, and care will be taken to ensure
that any other information from the interview that could identify you is not
revealed.
