2. Place of Residence in Each Member


Their house resides in a slum of Brgy. Tagabaca, Butuan City. Which their house is made of wood
and light materials. The floor area is approximately six sq. meters. The house is not well ventilated
as a result inadequate lighting. Breeding sites for mosquitoes, flies, cockroaches, and rodents are
inevitable due to poor environmental sanitation. Their toilet facility is located at the back of their
house, which they share with all the families in the compound.

3. Type of Family Structure


The A Family is a typical nuclear family consisting of the father, Mr. A, a construction worker.
Mrs. A is a full-time housewife and their one-year-old baby boy A.
4. Dominant Family Members in Terms of Decision Making Pertaining in Health Matters
For me it was very democratic and challenging since Mr. and Mrs. A don't have a hard time in
decision-making because they tend to consider each other's opinions first before coming up
with the final decision, especially regarding health matters.
5. General Family Relationship
They are both in a good relationship since they both select the right answer. While Mr. A is at
work , Mrs. A is careful in taking care of her child and her baby for the best; Based on my
understanding about the case given , they are trying their best to take care of the health of their
family even to be one though their situation is not the best.

According to the study , Pa-Bingo is their way of relaxation and entertainment since it is held in
their compound every month . And they have not participated in community groups as what
the neighbors.
C. ENVIRONMENTAL FACTORS

The room has only one bed that is wooden with a foam on top and they have an uncovered
cabinet that acts as the food storage.
d. Presence of Insects and Rodents
Breeding sites for mosquitoes, flies, cockroaches, and rodents are inevitable due to open drainage
and poor environmental sanitation.
e. Food Storage and Cooking Facilities
Their food storage is in an uncovered cabinet exposing it to germs and bacteria but they buy their
food in the market.
f. Water Supply
There is no water supply in the area, so Mr. A is forced to fetch water 20 meters away from their
house and costs 2 pesos per container. Their containers are more or less cleaned especially
covered.
g. Toilet Facility
The toilet facility is shared by people among the compound.It is unsanitary because some
people in the family cannot clean because of the lack of water supply.
h. Garbage and Refuse Disposal
Since there is a poor environment , The garbage is collected twice a week by a barangay truck.
i. Drainage System
There is an open drainage system which leads to poor environmental sanitation.
2. Kind of Neighborhood
The family resides in a slum barangay in Butuan City and their house is close also one thing
separates them is the wooden walls.
3. Social and Health Facilities Available
All the social and health facilities of Mr and Mrs. A are far away from home except the
Carinderia's and mini-market.
4. Communication and Transportation Available
Tricycles roam around as their means of transportation while public phones for communication
are available at the sari-sari store.

1. Medical and Nursing History of Past Significant Illness and Whom to Approach in Times of
Illness?
Mrs.A had a hard time delivering her first child due to hypertension. She gave birth at home
with the help of a "mang-hilotay "and plans to do the same with the second child. Her
husband, Mr. A, sometimes complains of severe pain at the back of his neck, maybe as a sign
of hypertension and over fatigue, but has no family history of hypertension. The family
doesn't use herbal medicine and goes to the health center when the need arises.
