2nd Semester A.Y. 2020-2021
I. COURSE DESCRIPTION:
This course covers the fundamentals, styles, form, and contents of simple legal forms. It
introduces students to the preparation of legal forms and techniques of legal writing.
II. LEARNING OUTCOMES:
 Learn the fundamentals, terminologies, styles, form, and contents of simple legal
forms;
 Know the techniques in legal writing;
 Prepare and write basic legal forms; and
 Apply the study in the criminology profession.
III. COURSE REQUIREMENTS:
The students are advised to conduct further research on the given topics in order to
substantially answer the learning activities, exercises, and questions on the modules.
The outputs/answers are to be submitted as scheduled either in hard copy (thru
personal or delivery service) or soft copy (in extreme and exceptional cases), whichever
is applicable and preferable to the students for consolidation prior to submission to the
professor. Soft copies should be sent thru the electronic/online platform exclusively
created for the course ([email protected]). An online written examination will
be given at the end of the course.
Assessment/evaluation will be in the following forms:
Performance Tasks/Exercises/Assignments/Project - 70%

Online Written Examination - 30%

Learning Outcomes:
 Gain and enhance knowledge and skills in legal forms writing particularly
different affidavits.
1. Research for a copy of the following affidavits and prepare the corresponding
legal forms involving a murder case:
a. Affidavit of Complaint
b. Affidavit of Witnesses
c. Affidavit of Arrest

Students shall complete and submit all the outputs on March 22, 2021 (Monday) using
the following format (paper size: A4; font style: Arial; font size: 12; spacing 1.15).
