IDENTIFICATION OF MOST CREDIBLE HAZARD IN BOF /SMS-II.

a) Personal Injury of Employee.


b) Fire incident.
c) Gas Poisoning.
d) Converter Puncture.
e) Ladle Puncture in BOF during operation.
f) Explosion in converter.

EMERGENCY PREPARDNESS

Introduction
Considering the possible emergencies at different activities at BOF, a well laid down
Emergency action plan needs to be worked out to contain and control potential situations.
Personal injuries, Fire & Explosions, toxic gas leakage from anywhere in plant may take
place. Natural calamities and workmen unrest may directly or indirectly aggravate the
conditions at complex.

Objective
The overall objectives of Emergency Plan for BOF is to provide guidelines to control
Emergency situations and to minimize its impact on personal, property and operation work
in an organized way.

Scope
This plan is applicable to all activities carried out at all process of BOF.

Emergency Scenarios and General Guidelines


Various scenarios that are anticipated to cause emergency at BOF are discussed in brief
along with general guidelines.

A. Personal injuries:
Inspite of putting best efforts some technical failure or human error may lead to personal
injury at site.

Guidelines for actions to be taken in case of personnel injuries are as under:

4. Quote your location, name and telephone No. while reporting any
accident/incident or a near miss State the nature of accident or incident. It is
equally important to report even when there is no injury or damage.
5. Inform to Safety Dept on 61134/7077764444 to Shift engineer.
6. Provide relevant First Aid if competent to do so / Rush the victim to the First
Aid Center.
7. Stay with the injured person until Medical Assistance arrives and a report of
the incident has been taken.

B. Fire Incident:

Considering the different operations in combustible materials and equipment’s


possible Fire / explosions at different areas may be occurred.
a) Fire in hydraulic room.
b) Fire in transformer, Electrical cables.
c) Fire in conveyor belt.
d) Fire in crane.
e) Fire during re-ladling.
f) Fire and explosion in COG line.
g) Fire in storage Bunkers of DRI.
h) Fire in storage container.
i) Fire in heavy automobiles (ForklIft , Skid Steer Loader, Gradal etc)
j) Fire in LPG online system.
k) Fire in Converter tilt drive area.
l) Fire in Ladle transfer Cars.
m) Fire in control rooms.

General guidelines for actions to be taken in case of fire are as under:

1. Whosoever is noticing fire should shout “Fire, Fire, Fire”.


2. Inform the persons nearby you. Also contact BOF Control Room on Telephone
no. 61364
3. Contact Security Control Room on Telephone no. 103/7077756111 Fire
Control Room on Telephone no. 101/7077756141 for any help.
4. Give your Location, Name and speak slowly, clearly and calmly.
5. Do not be panic; there is always sufficient time for an emergency telephone
call.

6. Try to put out the fire with the nearest suitable portable Fire Extinguisher if
you know how to operate.
7. Go to the nearest area and try to inform more people for any help.
8. Walk, do not run, and follow the shortest escape route if it is totally not safe
to do so.
9. Do not make any attempt for collecting valuables or personal goods.
10. If it is inside the running unit after informing the workmen rush to the
designated assembly point (select as per the wind direction). Assembly point
for BOF is designated at Weigh Bridge front and Assembly point for SMS-II is
designated at SMS-II canteen square.
11. Under no circumstances put your own Life at risk.
12. If the Fire appears too large to tackle with fire extinguishers or available
resources, leave it to the qualified Fire Fighting Services.
13. Leave the Building / Site in an orderly manner to the designated assembly
point.

C . Gas Poisoning:
a) CO gas poisoning in confined space. (Scrubber, Converter, Hopper etc.)
b) CO gas poisoning due to toxic leakage at different places like launder line,
Switch Over Station, BOF duct, lance changing platform, seal pot area, and
Ladle preheater areas etc.)
c) COG networks lead to Emergency situation at complex.

Guidelines for actions to be taken in case of such situations are as under:


1. Evacuate all the personnel from the area immediately depending upon the
wind direction.
2. Move upwind if release is downwind of you and move crosswind if release is
upwind.
3. Call for help from fire and medical by phone.
4. Instruct Time Office people for head count.
5. In case of causality remove victim to a safer area and apply artificial
respiration if required.
6. Arrange transport of victim to medical aid Centre.

D. Converter Shell Puncture:


a. During Hot metal charging
b. During Blowing
c. During Tapping

a. During Hot metal charging :


• If the metal trickling or shell puncture observed on the converter shell ,
stop hot metal charging immediately, move away the hot metal ladle from
the converter.
• Tilt the converter to the tapping side, Based on the damage severity,
further action will be take by Area incharge and HOD .
b. During Blowing:
• If the metal trickling or shell puncture observed on the converter shell ,
stop the blowing process , observe the location of the shell damage or
trickling part.
• Tilt the converter the converter to the opposite side of the shell effected
area (If shell damage or trickling is observed on charging side , tilt the
converter to tapping and If shell damage or trickling is observed on Tapping
side , tilt the converter to charging side).
• Overall, the converter to position such that the liquid metal is away from
the damaged part of shell.
• Based on the affected zone of the converter , the liquid metal to be drained
out of converter into the steel ladle either by converter mouth or tapping
hole .
c. During Tapping :
• If the metal trickling or shell puncture observed on the converter shell ,
stop the tapping process and lift the converter to the charging side ,
observe the location of the shell damage or trickling part.
• Tilt the converter the converter to the opposite side of the shell effected
area (If shell damage or trickling is observed on charging side , tilt the
converter to tapping and If shell damage or trickling is observed on Tapping
side , tilt the converter to charging side).
• Overall, the converter to position such that the liquid metal is away from
the damaged part of shell.
• Based on the affected zone of the converter , the liquid metal to be drained
out of converter into the steel ladle either by converter mouth or tapping
hole .

E. Puncture of ladle in BOF during operation.

Puncture of ladle during BOF during or after tapping operation, may also lead to some
emergency & damage to property.
Guidelines for actions to be taken in case of such situations are as under:
1. Stop the tapping practice by lifting the converter to up right position.
2. Stop the Argon purging of the ladle.
3. Evacuate all the personnel from the area immediately depending upon the fire
caused due to spillage of metal.
4. Inform shift manager & area in-charge soon.
5. Shift manager will inform to the HOD- operation and HOD-Plant, fire
department is also to be informed for firefighting if any.
6. Move the ladle immediately to the crane approach.
7. Coupling/Decoupling of argon hose in puncture ladle is strictly prohibited. The
ladle to be lifted at the cost of damage of hose and MS line.
8. Arrangement should be made immediately for re-ladling. Ready for operation
ladle (Ladle slide gate closed and filled with nozzle powder in well block) to be
placed Prepared stand by ladle to be lifted from horizontal ladle preheating
stand for re-ladling.
9. If the shell puncture is towards the ladle tilter, The auxiliary hoist of crane to
be engaged in tackle tilter to further engage the ladle tilting hook for reladling
so that any metal stream does not damage the wire rope of EOT crane. The
tackle tilter is placed near BOF 1/2 (0 – 0 meter).
10. The punctured ladle must not hanged above LTC. It should be displaced nearby
area where no possibility of further fire catching or equipment damage.
11. Make a Ladle ready for reladling of punctured Ladle’s metal as early as
possible. Any Ladle with or without sand filled in its nozzle with Slide Gate in
close position can be used.
12. Safe Area is to be identified for reladling.
13. Reladling decision & identification of ladle for reladling will be taken by LF shift
in charge & ladle Engineer.
14. Parallel to reladling, any fire on LTC or other equipment to be extinguished and
to be handed over to MMD/EMD to check equipment condition and to be
made operational, if any damage occurred.
15. In case of any burn please follow the process for personal injury mentioned in
(Para-A) and normal water to be applied on affected limb for 15 to 20 minutes.
Take the victim to shower room for normal water bathing.
16. Arrange to clear the metal from the track and nearby area.
17. Assess the damage and arrange to restore by electrical, mechanical &
operation.
18. Normalize the situation and take cold trail.

F. Explosion.
1) Due to leakage of lance explosion may took place inside convertor.
2) Water leakage from Hood/Skirt during blowing.
3) During taping of heat at Converter furnace.
4) Explosion due to Wet scarp, liquid slag/metal in the converter during hot
metal charging.
5) Explosion in COG line.

Guidelines for actions to be taken in case of such situations are as under:

1. Evacuate all the personnel from the area immediately.


2. Due to water leakage from lance as per logic, blow gets terminated by
leakage fault generation. If observed that the water leakage is heavy and
water is going into the bath, immediately close the respective line valves and
close the bottom stirring media valves through HMI. Don’t tilt the converter
till the water is evaporated. Ensure the water in the converter by visual
inspection from the lance dome.
3. Due to water leakage from Hood or skirt, stop the blow immediately. If
observed that the water leakage is heavy and water is going into the bath,
immediately close the respective line valves and close the bottom stirring
media valves through HMI. Don’t tilt the converter till the water is
evaporated. Ensure the water in the converter by visual inspection from the
lance dome. If water is not going into the converter, tile the converter to
charging side or tapping side (Opposite side to the water leakage zone ).
4. During hot metal charging, if any Abnormal sound observed, stop the hot
metal charging, rock the converter 2 time and ensure that no liquid
metal/slag is available in the converter. If liquid slag/metal exists, add fluxes
and make the liquid slag/metal to solid state by rocking the converter till it
gets solid.
5. Leave the Building / Site in an orderly manner to the designated assembly
point.
6. Inform the persons nearby you. Also contact to control room on Telephone
no. 7077762200 (BOF/Dispatcher).

Control Room on Telephone no. 101/7077756141 for any help.

13. If it is inside the running unit after informing the workmen rush to the
designated assembly point (select as per the wind direction). Assembly point
for BOF is designated at Weigh Bridge front and for SMS-II Assembly point is
designated at canteen square.
14. Under no circumstances put your own Life at risk.
15. Instruct Time Office people for reporting the day’s attendance and match
with the head count.
16. In case of causality remove victim to a safer area and apply First Aid by the
authorized person.

Communication system

An important key to effective emergency response is an effective communication


system. At BOF and SMS-II intercom, mobile, walkie-talkie and megaphones will be
used for communication during emergency.

Emergency Control Center

To co-ordinate with emergency situation at BOF/SMS-II, BOF Control Room shall be


operated as an Emergency Control Center. Site main controller will co-ordinate from
this control room. Telephone No. 7077762200. Control room shall be equipped with
necessary communication facilities, documents, drawing, Emergency kits and
Computers etc.

Role & Responsibility

Responsibility of Shift Manager

1. Any person noticing any emergency shall immediately shout for “Help” and report
to shift Manager and he shall immediately to inform fire control room on telephone
no 101/7077756141 and security control room on telephone No.103/7077756111.
2. Shift Engineer shall inform departmental head and other as per information chart
and try to minimize the impact of emergency with resources available.
3. Shift Manager shall also try to extinguish the fire and assist injured as possible.
4. Shift Manager shall instruct security and ensure that all non-essential employees are
moved to a safe assembly area and organize head count if required.

Operation Head- Incident Controller

1. On noticing or receiving message of an emergency from field, rush to the


emergency site and act as an Incident controller.
2. He will be the overall in-charge on all matters related with declaration of emergency,
fire fighting, rescue, calling for assistance, transportation, evacuation, all clear signal
etc. (Siren Signaling System: Declaration of emergency: 20 Second up ON and 10 Seconds
down OFF for 4 times and All clear: for 1 Minute continuously)
3. He will assess the situation and take necessary actions to control the emergency
and give necessary instructions to field personnel.
4. Ensure safe evacuation of field personnel, if required. Liaise with Site Main
Controller, Firefighting crew, Medical/Ambulance, Safety and security services.
5. He shall be in contact with Site main controller, keep him informed about site
situation and seek directives for mitigation of emergency.
6. Instruct fire officer for fire control and rescue measures.
7. Instruct security to barricade the affected areas downwind. Arrange for
transporting of casualties if any.
8. Keep in constant touch with shift Manager and fire Dept.

Departmental Head – Site Main Controller

1. Provide leadership and guidance to the major emergency management team.


2. Immediately on receiving message regarding emergency he will rush to BOF
Emergency Control room.
3. He shall be in regular touch with Incident controller and guide him accordingly.
4. He shall be in regular touch with plant emergency command team as per the
requirement of emergency situation.
5. Inform and appraise at regular intervals regarding the situation to the top
management.
6. Take strategic and technical decisions in line with the accident scenario to
terminate the cause of emergency.

Fire Department Crew

1. Respond to the emergency call with fire crew in first turn out.
2. Report to Incident controller at site of emergency.
3. Decide line of control actions in consultation with Operation head-BOF
(Incident Controller)/ Department head (Site main controller) for firefighting
and other control actions including evacuation of trapped personnel.
4. Deploy Auxiliary fire team for assisting fire crew.
5. Ensure safety of the crew members.
6. Keep constant contact with shift Manager /Operation head / Department head/
Fire control room.

Security Officer

1. Instruct and deploy security personnel at security posts / gates and around the
affected area.
2. Mobilize the auxiliary security team at emergency site immediately on receiving
emergency call.
3. Cordon off the affected area and guide traffic / emergency vehicles and control
unnecessary gathering of persons around.
4. Ensure vigilance at security posts / gates and be in constant contact with
operation head/department head and other members connected with
firefighting or rescue operation.
5. Mobilize for evacuation of personnel from safe assembly points to safe
locations.

Medical Center

1. Upon receiving message of an emergency, anticipate the casualty and prepare


accordingly as per medical response plan. Inform Medical Officer.
2. Send the Ambulance to incident site along with necessary staff.

FIRST AID IN CASE OF GAS EXPOSURE


Take the victim away from the gas affected area to the fresh air (opposite to the wind direction).
Loosen the tight fitting clothes, shoes and belt so that, victim can breathe comfortably.
Ask the victim to take longer breath (if conscious).
Give Oxygen to the victim if possible.
Provide artificial respiration (if required).
Send the victim to nearest Medical Centre for medical aid.
Inform Gas Safety to take sample of the area.
