CONSENT AGREEMENT

The Sangguniang Barangay of Handong, Libmanan, Camarines Sur headed by Punong Barangay
Nestor A. San Andres Sr. executes the CONCRETE PATHWAY PROJECT at Barangay Handong,
Libmanan, Camarines Sur with appropriation amounting to Php 376,448.80 under 20% Local
Development Fund of Calendar Year 2022. The project starts at the month of September 2022
and expectedly be completed on January 30, 2023 under F.N. MAYOR CONSTRUCTION AND
SUPPLY.

I, NESTOR A. SAN ANDRES SR., Punong Barangay of Handong, Libmanan, Camarines Sur hereby
claim that the following names appear below had been notified by the Sangguniang Barangay
to be informed that there will be undergoing CONCRETE PATHWAY PROJECT at their respective
Zone and be executed at the existing pathway in their private premises.

The following names, bona fide residents of Barangay Handong, Libmanan, Camarines Sur,
claimed to be notified and hereby confirm to agree and consent the CONCRETE PATHWAY
PROJECT be executed at their private premises with existing awarded right-of-way to public
with support and cooperation.

Nestor A. San Andres Sr. executes the INSTALLATION OF SOLAR STREET LIGHTS at Barangay
Handong, Libmanan, Camarines Sur with appropriation amounting to Php 320,000 under 20%
Local Development Fund of Calendar Year 2022. The project starts at the month of September
2022 and expectedly be completed on January 30, 2023 under F.N. MAYOR CONSTRUCTION
AND SUPPLY.

I, NESTOR A. SAN ANDRES SR., Punong Barangay of Handong, Libmanan, Camarines Sur hereby
claim that the following names appear below were notified by the Sangguniang Barangay to be
informed that there will be undergoing INSTALLATION OF SOLAR STREET LIGHTS at their
respective zones and be executed along their private premises.

The following names, bona fide residents of Barangay Handong, Libmanan, Camarines Sur,
claimed to be notified and hereby confirm to agree and consent the INSTALLATION OF SOLAR
STREET LIGHTS be installed at their private premises with support and cooperation.

of 10 meters and with appropriation amounting to Php 320,000.00 under 20% Local
Development Fund of Calendar Year 2022. The project starts at the month of September 2022
and expectedly be completed on January 30, 2023 under F.N. MAYOR CONSTRUCTION AND
SUPPLY.

I, NENITA A. BUSTAMANTE had been notified by the Sangguniang Barangay to make me


informed that there will be undergoing INSTALLATION OF SOLAR STREET LIGHTS at our Zone
and be executed in front or within my premises.

I hereby agree and consent the foregoing INSTALLATION OF STREET LIGHTS be executed at my
private premises with support and cooperation.
