Q-1 Safety requirements at working at height.


Ans. Work at height means work in any place where, if there were no precautions in
place, a person could fall a distance liable to cause personal injury.
 All work at height is properly planned and organized
 A risk assessment is carried out for all work conducted at height
 Appropriate work equipment is selected and used
 People working at a height are competent
 Equipment used for work at height is properly inspected and maintained
 Risks from fragile surfaces are properly controlled 
 Where fall protection is not sufficient, employers may provide personal fall arrest
equipment such as harnesses that reduce the chance of injury or death if the
employee does fall. Other fall protection methods include safety nets, stair
railings, and handrails.
 Fixed ladders on towers, water tanks, and chimney ladders that are over 20 feet
in unbroken length may be equipped with safety devices such as lifebelts, friction
brakes, and sliding attachments instead of cages.
 Employers should keep facility floors in clean and dry condition to prevent
workers from slipping and falling. Employers should keep facility areas that are
subject to high traffic free from tools, materials, debris, and spilled liquids.
Q-2 Working at confined space.
Ans. A confined space is one which is both enclosed and which also has a reasonably
foreseeable risk to workers of fire, explosion, loss of consciousness, asphyxiation or
drowning.
Dangers can arise in confined spaces because of:
 Lack of oxygen
 Poisonous gas, fume or vapour
 Liquids and solids suddenly filling the confined space, or releasing gases into it
when disturbed
 Fire and explosions
 Residues left behind which can give off gas, fume or vapour
 Dust
 Hot working conditions
If you cannot avoid entering into confined space then following points should be
considered before working:
 Competence, training, supervision and suitability
 Permit-to-work procedure
 Gas purging and ventilation
 Check for dangerous residues
 Testing and monitoring of the atmosphere in confined space
 Mechanical, electrical and process isolation before entering the confined space
 Respiratory protective equipment should be provided
 Other personal protective equipment should be provided
 Work equipment to be used in confined space should be used safely
 Communications of hazards should be there
 Access and egress for entering or for coming out at time of emergency
 Check for flammable or explosive atmospheres
 Check for presence of combustible materials

Q-3 Work Permit System


Ans. A safe work permit is a written record that authorizes specific work, at a
specific work location, for a specific time period. Permits are used for
controlling and co-coordinating work to establish and maintain safe working
conditions.
The implementation of this system will help in bring down the risks at work sites to
acceptable level, thereby reducing possibility of any accident, fire, explosion, property
damage and adverse effect on environment.
The objectives of the Work Permit System are to exercise control over the maintenance,
repair and construction activities by assigning responsibilities, ensuring clear cut
communication between interested functions & safety considerations to the job, its
hazards & the precautions required.
It ensures that the work is properly defined, authorized, operating personnel are aware
what is going on, precautions to be taken are specified and the persons executing the
job understand the nature and extent of hazards involved.
Safe Work Permits are usually made out in three copies:
 One retained by the issuer,
 The second for the workers directly involved in the task and
 The third for the safety department
Mainly there are 6 types of work permits that are generally issued:
 Cold work permit
 Hot work permit
 Confined space work permit
 Electrical work permit
 Excavation work permit
 Working at height permit
Work permits include information regarding following points:
 Basic information like requested by, requested for which work, date of request,
description of job, etc.
 Control measures like PPEs, compatibility of person working in given job,
atmospheric concentrations of contaminants present, etc.
 Validation like sign of approving authority, date and time of issue, date and time
till its valid etc.
 Information regarding personnel involved in work
 Information regarding withdrawal of permit like PPEs and tools submitted back,
sign of approver, date and time of withdrawal
