English as a Second Language (ESL) Tutor Contract

This contract outlines the terms and conditions for tutoring services provided by teacher
PRESYBELLE LEBIG with Professional Regulation Commission (PRC) License No. 1876372 to Mr./Ms.

Teacher’s Responsibilities

Teacher will prepare ESL lessons for tutoring sessions designed to help Student make progress in the
areas agreed to by Student and Tutor.

Teacher agrees to provide ESL lessons to Student according to the Tutoring Schedule listed below. Tutor
agrees to be on time for each lesson. Tutor will wait for Student at least 20 minutes in the event that
Student is late.

The teacher should extend time equivalent to the number of minutes she is late.

The teacher should inform 2 hours before the class if she has an emergency and cannot attend the class.

Teacher will give homework every after class and will conduct assessment test every end of the month.

Student’s Responsibilities

Student should be on time for each lesson. Student will wait for the teacher at least 15 minutes in the
event that Tutor is late.

Student will make every attempt to complete any homework agreed to by Student and Tutor.

Student should inform the teacher 2 hours before the class if he has an emergency or can’t attend the
class. Failure to do so will consider the session as paid.

Tutoring Schedule

Student and Tutor both agree to meet at trice a week, unless unforeseen circumstances make it
impossible. Tutor and Student will meet for lessons on the following day(s) and at the following time(s):

Day: TUESDAY/WEDNESDAY/THURSDAY Time: 8:00-9:00 P.M VNT

This schedule may be modified when needed and as agreed to by both Student and Tutor.
Course Fees

PRESYBELLE LEBIG $200 USD for 28 session of classes with the duration of 1 hour per session with 1
free session sent via Paypal with the reference No. .

This course was paid on the day of , year and effective starting on the 5th day
of January, year 2023.

If Student needs to cancel a scheduled lesson, Student should contact the teacher at least 2 hours
before the scheduled lesson. Student may contact teacher by email or skype. Student and Tutor should
agree to reschedule the canceled lesson as soon as possible.

The total hour of the entire course is 28 hours with 1 free lesson.

START DATE: January 05, 2023

Level assessment and report card

The tutor must send a report card to the student every end of the month to monitor the progress of the
student.
