Introduction
Definition:

A laboratory test in which blood, taken from the patient,

is inoculated into bottles containing culture media to determine whether
infection-causing microorganisms (bacteria or fungi) are present in the
patient’s bloodstream.

Aim:
- Confirm infectious aetiology
- Identify the etiological agent
- Guide antimicrobial therapy
Indications Complications
● Sepsis ● Hematoma
- Shock, fever, chills, rigor, tachycardia, ● Fainting
tachypnea, decrease urine output ● Infection

● Endovascular infection
- Vascular graft, history of IE, prosthetic valve,
pacemaker, intravascular catheter infection

● Local infections
- Vertebral osteomyelitis, meningitis, epidural
abscess, septic arthritis, pneumonia

● Leukocytosis/ leukopenia
Site selection
Preferred site:
● Median cubital vein in antecubital
fossa commonly used for
venepuncture
● Other upper extremity blood vessels

Prohibited sites:
● Veins proximal to existing IV cannula
● Areas of broken skin or bruising

To identify a vein:
● Push the vein firmly with index finger
and slowly release the pressure
● The vein will bounce back
Equipment
● Apron
● Non-sterile gloves
● Tourniquet (single-use)
● Blood sampling device with blood
culture bottle adapter (e.g. winged
blood collection set)
● Blood culture bottles x 2 (anaerobic
and aerobic):
● Sharps container
● Cleaning swab x 3 (2% chlorhexidine in
70% isopropyl alcohol)
● Sterile gauze
● Sterile plaster
● Tape
● Laboratory forms, labels and
transportation bag
Technique
Pre-procedure basics: 1. Prepare bottles for inoculation
● Greet the patient ● Wash hands with soap
● Introduce yourself and water then dry
● Explain the procedure ● Remove the plastic flip-cap
● Gain consent from the bottles
● Confirm patient’s identity ● Disinfect the septum
● Gather all required materials before ● Allow bottle tops to dry to fully disinfect
beginning collection process ● Don gloves and apron

2. Prepare venipuncture site

● Clean with soap and water if skin is
visibly soiled
● Apply a disposable tourniquet
● Palpate and identify a vein using clean
examination gloves
● Cleanse puncture area using appropriate
disinfectant
● Allow disinfectant to fully evaporate
Technique
3. Venipuncture
4. Culture bottle inoculation
● Attach a winged blood collection set to
● Place the adapter cap over aerobic
a collection adapter cap
bottle and press straight down to pierce
● Do not re-palpate the prepared vein
the septum
before inserting the needle
● Hold the bottle upright, below the draw
● Insert the needle into prepared vein
site and add up to 10 mL of blood per
● At 30૦ angle or less, bevel facing
adult bottle and up to 4 mL per pediatric
forwards
bottle
● Observe for flashback of blood
● Repeat procedure for anaerobic bottle
Technique
5. Other blood tests

● If blood is being collected for other tests, an

insert placed into the adapter cap may be
required. The insert will guide blood collection
tubes into the needle.
● If other blood tests are requested, always
collect blood culture first.

6. Finish the procedure

● Release the tourniquet and apply gauze, then
remove the butterfly needle
● Discard winged collection kit into sharps
container
● Remove gloves and perform hand hygiene
● Labels the bottle carefully and correctly
● Send blood sample to lab (within 2 hours)
● Thank the patient
