Introduction
Definition:
Aim:
- Confirm infectious aetiology
- Identify the etiological agent
- Guide antimicrobial therapy
Indications Complications
● Sepsis ● Hematoma
- Shock, fever, chills, rigor, tachycardia, ● Fainting
tachypnea, decrease urine output ● Infection
● Endovascular infection
- Vascular graft, history of IE, prosthetic valve,
pacemaker, intravascular catheter infection
● Local infections
- Vertebral osteomyelitis, meningitis, epidural
abscess, septic arthritis, pneumonia
● Leukocytosis/ leukopenia
Site selection
Preferred site:
● Median cubital vein in antecubital
fossa commonly used for
venepuncture
● Other upper extremity blood vessels
Prohibited sites:
● Veins proximal to existing IV cannula
● Areas of broken skin or bruising
To identify a vein:
● Push the vein firmly with index finger
and slowly release the pressure
● The vein will bounce back
Equipment
● Apron
● Non-sterile gloves
● Tourniquet (single-use)
● Blood sampling device with blood
culture bottle adapter (e.g. winged
blood collection set)
● Blood culture bottles x 2 (anaerobic
and aerobic):
● Sharps container
● Cleaning swab x 3 (2% chlorhexidine in
70% isopropyl alcohol)
● Sterile gauze
● Sterile plaster
● Tape
● Laboratory forms, labels and
transportation bag
Technique
Pre-procedure basics: 1. Prepare bottles for inoculation
● Greet the patient ● Wash hands with soap
● Introduce yourself and water then dry
● Explain the procedure ● Remove the plastic flip-cap
● Gain consent from the bottles
● Confirm patient’s identity ● Disinfect the septum
● Gather all required materials before ● Allow bottle tops to dry to fully disinfect
beginning collection process ● Don gloves and apron
