    Reference: Incident Record Form No: 2022-06-008 dated June 06, 2022.

    On or about 02:00 PM of June, 06 2022 complainant Mary Jane P. Fajardo, 25 years


    old, single of Ayala, Zamboanga City appeared and reported to this station that certain Jerwil
    Casumpa, 31 years old, single and a resident of Ayala, Zamboanga City alleging that on the
    same day at 10:00 AM in the morning, the latter raped her while she was unconscious. This
    report was formally recorded under Incident Record Form (IRF) No. 2022-06-008 and
    subjected for further investigation by the undersigned investigator.
    Initial investigation conducted disclosed that on August 6, 2022 at 10:00 in the
    morning, complainant Mary Jane P. Fajardo (Mary Jane, for brevity), 25 years old, single, and
    a resident of La Paz, Zamboanga City was taking a bath at their home when she heard a loud
    and persistent knock on the main door of their house. She decided to check out thinking that it
    was urgent and that no one can open the door since her parents and her brother were at their
    farm.
    She wrapped herself with a towel and asked who the person outside was. At the door
    he asked who the person outside was, it turned out to be Jerwil Casumpa (Jerwil, for brevity),
    31 years old, single and a resident of Ayala, Zamboanga City, the one who regularly delivers
    fresh carabao milk. She recognized his voice. When she opened the door a little, Jerwil insisted
    to enter the house to deliver the four bottles of milk. With much hesitation, the Mary Jane
    allowed him to enter. Jerwil then insisted that she taste the milk he is delivering, he filled the
    empty glass with milk. Mary Jane hurriedly but after consuming half of the glass she felt dizzy
    and lost consciousness.
    When she regained consciousness, she was already lying on the floor naked while her
    brother was shaking her to wake her up. When she woke up she felt pain all over her body
    especially her private part. (See attached Judicial Affidavit of Complainant MARY JANE P.
    FAJARDO) Mary Jane’s neighbor, Sharlene Fair Sumalpong, who came from the market saw
    Jerwil leaving the house of Mary Jane thereafter. She greeted Jerwil but the latter only nodded
    at her. (See attached Judicial Affidavit of Witness SHARLENE FAIR SUMALPONG).
    The undersigned investigator assisted the complainant (MARY JANE P. FAJARDO)
    to undergo Medico-Legal Examination at Zamboanga City Medical Center in order to establish
    the veracity of their complaint and to form part of tis investigation. Complainant was physically
    examined by Dr. Wafa Mina R. Mariwa, government physician detailed at Zamboanga City
    Medical Center on June 07, 2022.
