    AFFIDAVIT OF CLOSURE OF BUSINESS

    I, DAR ANDREW CAYABYAB, of legal age, Filipino citizen and presently


    residing at 30 Merryland Street Brgy. Pinagkaisahan, Cubao Quezon City and
    hereby represented by my authorized representative WINNIE HERNOSA LASIGAS, of
    legal age, Filipino citizen and presently residing at Amihan Road Brgy. Santo
    Cristo, City of San Jose Del Monte, Bulacan, after being duly sworn to in accordance
    with law do hereby deposes and says:

    1. That I am the proprietor of MINESKI FRANCHISE CORPORATION with


    business address at M. Ponce BLDG., Plaza Naning Brgy. Poblacion,
    Baliwag, Bulacan;

    2. That I am applying for the termination and/or cessation of the business


    operation of said business effective on 31 March 2020 due to bankruptcy
    during the pandemic;

    3. That the above-mentioned business is now closed effective 31 March


    2022 due to bankruptcy during the pandemic;

    4. That MINESKI FRANCHISE CORPORATION has no outstanding liability


    and obligation; and

    5. That each and every fact herein stated is true and I am willing to testify
    before any court of justice to substantiate the truth of each and all the
    said facts.
