DECLARATION
For 12months Sea Service

I, the undersigned Cdt.______________________, (Course) ____________together with my


Parents/ Guardian Mr./Mrs.__________________________ ___________hereby declare the
WE FULLY UNDERSTAND the following topics discussed during PEOS:

I. DOCUMENTS:

i. The approved Seagoing Service requirement shall be enrolled in the same Maritime
Higher Education Institution (MHEI)
ii. The 12 months structured seagoing service shall be enrolled prior to embarkation
iii. The SIRB (Seaman's Book) is mandatory requirement therefore all the details required
to be filled in the SIRB shall be complied such as the vessel's gross tonnage, kilowatt,
date and place of embarkation and disembarkation, rank (cadet or apprentice) stamp
and Master's signature etc. Any erasures/corrections shall be subject for query.
iv. The Company Sea Service shall reflect the data in the SIRB especially the dates. Data
such as name of the vessel, rank (cadet or apprentice), type of vessel, gross tonnage
and Kilowatt must coincide with the other documents.
v. A Certificate of bridge/engine watchkeeping stating the watchkeeping duties
performed for 180- 250 days only or shall be requested from the shipping/manning
company.
vi. In conjunction with requirement above (part E), the Daily Journal of Bridge
watchkeeping duties for deck cadets and Daily Journal of Engine-Room watchkeeping
duties must also be complied. See STCW CirC. No. 2016-01
vii. If I (the cadet) shall embark onboard an international trade vessel, I should make sure
that my Passport has the immigration stamp of my departure and arrival in the
Philippines.
viii. If I (cadet) shall embark onboard a domestic trade vessel, I should make sure that I
secure an original copy of the Crewlists on the day I embarked and on the day I
disembarked with ship's stamp and Coast Guard stamp on each document.
ix. The Confidential Evaluation shall be completed by the Master for deck cadet or Chief
Engineer for engine cadet prior disembarkation.

II. TRAINING RECORD BOOK AND SEA PROJECTS

i. The use of the approved Training Record Book (i.e. ISF, Global Met and other
approved TRB) is compulsory. Approved TRB means;
a) Front page of the TRB has the name of the issuing Maritime Higher Education
Institution and the cadet's TRB code/control number duly signed by the
OBTO.
b) All pages of the TRB must be stamped by the OBTO with the cadet's very own
TRB code/control number.
c) TRB bought outside the school shall be submitted back to the UCLM OBTO
for stamping and assigning of the cadet's own TRB code/control number
prior embarkation

For ISF TRB and Global Met


ISF TRB- Deck and Engine 1) Section 1 Guide to Completion should be read and
Global Met- Engine understood by the Master, Chief Engineer, Designated
Training officer onboard and the cadets.

1.1) Section 1 Introduction should be read and understood

2) Section 2 Summary Record of Progress, if type of vessel

2.2) Roles and Responsibilities should be read and

3) Section 3 Mandatory Safety and Shipboard
ISF TRB- Deck and Engine Familiarization is good for 6 ships, all vessels declared to
Global Met- Engine comply the 12 months shipboard training must be reflected
on this section. The Master or Chief Engineer of each vessel
should sign the last part of this section.
3.1) General Information, should be read and understood
Global Met- Deck
by both cadet and Designated Training Officer onboard.

ISF TRB- Deck and Engine 4) Section 4 Particulars of Ships should be filled up
Global Met- Engine diligently. Corrections should not be erased with white ink
but should be corrected accordingly as an officer/engineer

4.1) Task Progress Overview, if type of vessel permits, at

5) Section 5 Safety at Work for engine and COLREGS for

6.1) Section 6 Ship’s Particular should be filled up


Global Met- Deck diligently. Corrections should not be erased with white ink

7) Section 7 Tasks for Officers in Charge — Competences


that are not applicable onboard should be marked N/A.
ISF TRB- Deck and Engine
Competences that are signed by the designated training
officer will be subjected to further assessment. Make sure
that the column with the COMPETENCE DEMONSTRATED

on the far right corner of the pages will be signed last, once
all tasks under the competence are completed.

Global Met- Deck 7.1) Section 7 International Regulations for Preventing


Collisions at Sea should be 100% carried out.

7.2) Section 7 Assignments and Section 8 On Board


Training Assignment Record, Failure of submitting these
requirements will further delay B.S. Application for
Global Met- Engine
Endorsement Certificate until the Assessors and the OBTO
are fully satisfied of your submission. Sea Projects will be
subjected for assessment during B.S. Application

For ISF TRB and Global Met


8) Section 8 for Deck Cadets- Steering Certificate, the
ISF TRB- Deck
master should sign the Cadet’s Steering Certificate after
completion of the tasks under the competence.

8.1) Section 8 Project work for Engine cadets, Failure of


submitting these requirements will further delay B.S.
Application for Endorsement Certificate until the Assessors

10.1) Section 10 Project Work, Failure of submitting these

Global Met- Deck Endorsement Certificate until the Assessors and the OBTO
are fully satisfied of your submission. Sea Projects will be
subjected for assessment during B.S. Application

The objectives of carrying out projects during sea service is to ensure that I will gain knowledge
of the operation of the ship in which I served and familiarize the use and maintenance of the
equipments onboard such as the Life Saving and Fire Fighting Appliances.

I am required to report to the Onboard Training Office the progress of my training and the use
of ISF On-Board Training Record Book , Global Met and other approved TRB every three
(months), by sending a scanned copies of the pages of tasks and Sea Project that has been
completed and approved by the Master or the designated training officer or any other concerns
to the UCLM OBTO official email address [email protected]

Email Subject Format: CADET FULLNAME (Family name first) MONTH & YEAR or CAR / DATE
EMBARK / NAME OF SHIP / C (for coastwise) or W (worldwide) 1, 2,3,4 (quarter report)

III. B.S APPLICATION

a) I am required to report to the Onboard Training Office immediately after I disembark or


after I complete my 12 months shipboard training.

IV. JOINT CHED-MARINA MEMORANDUM CIRCULAR NO.1 SERIES 2019

a) For clarification of the Onboard Training and B.S. Application procedures, standards are
being imposed by CHED under the Annex B of Joint CHED-MARINA Memorandum Circular
No. 1 Series 2019
