Subject : Proposal for Geotechnical Investigation

PROPOSED SPVC PLANT EXPANSION
STA. ROSA, LAGUNA

Dear Sir:

With reference to the above subject, we are pleased to submit our geotechnical investigation proposal for
your evaluation and consideration.

A. SCOPE OF WORK

1. Drilling of Two (2) borehole with minimum depth of 15.0 meters. The boreholes shall be drilled at the
Client’s specified/marked locations. However, this assumption may change depending on the actual site
condition such as relative areas, height and distances between areas that are to be drilled upon final
confirmation from Client’s representative.

2. Conduct Standard Penetration Test (SPT) including disturbed sampling every 1.50-meter interval
including water table monitoring/recording for each borehole only.

3. Conduct laboratory testing on selected soil and rock samples taken from the boring/coring as follows;

a) Natural moisture content ASTMD 421

b) Sieve analysis ASTMD D-422
c) Atterberg Limits ASTM D-4318
d) Unified Soil classification System ASTM D 2487
e) Unconfined Compression test ASTM D2166
4. Preparation and submission of three (3) copies of geotechnical report signed/sealed by our geotechnical
engineer/evaluator which will contain the following:

a. Soil profile, type and classification, SPT N-values and water table location for test holes if
encountered. Stratification of the area explored is limited through the drilling depth explored.
b. Discussion on soil stratification and other technical information relevant to the design of the
structural foundation within the area.
c. Evaluation and recommendation of the soil bearing capacity for different footing sizes and depths
for shallow and deep foundation.
d. Evaluation of site liquefaction potential should the site be susceptible to this phenomenon
f. Photos of soil exploration and soil samples per test hole.

B. COST PROPOSALS

For the above scope of work our fees are as follows:

I. Field Work (2 Borehole at a maximum of 15.0 meters depth)…………Php. 45,000.00 per Borehole

1. Mobilization/Demobilization…………………………………………..Php. 20,000.00
2. Drilling through ordinary soil
3. Drilling Through Hard Strata
4. Standard Penetration Test (SPT)
5. Additional Borehole per meter depth (Php. 2,500 / meter depth)

II. Laboratory Works

III. Geotechnical Evaluation Report
(3-sets-signed/sealed)

Total: Php 110,000.00

Add Vat: Php. 13,200.00
Grand Total: Php. 123,200.00
B. TERMS AND CONDITIONS

1. Works shall commence upon receipt of Notice-To-Proceed (NTP) or Job Order (JO) with
corresponding fifty (50%) percent down payment of the total project cost.
2. The client shall provide site development plan with location of boreholes and shall locate and
mark test/boreholes jointly with FDCT. To secure permit if necessary, facilitate gate passes
needed by our field crews to access the site.
3. The client shall allow our crew to stay at the site and provide security to ensure safety of our
raft, equipment, tools and personnel/staff, a temporary area/facility with water, power, a comfort
room “free of charges” during the entire duration of drilling or field works as these were not
charged either.
4. Test holes shall be free of any obstructions and concrete cover prior to our mobilization. FDCT
shall not in any way be held liable for any accident or damage in the drilling of the
located/marked boreholes.
5. Mode of Payments:
a. Down/Advance payment: Fifty percent (50%) of the total price.
b. Full payment: upon submission of final geotechnical report

6. Completion time: Thirty (30) working days (site drilling works laboratory evaluation)
7. Acceptance of this proposal shall constitute a complete and binding contract between us.

Trusting the above subject proposal meets your favorable approval. Should you have any clarifications,
please feel free or don’t hesitate to contact us through mobile numbers 09225087562 / 090178617719
