PROCEDURE CHECKLIST

ANTHROPOMETRIC MEASUREMENTS AND WEIGHING

Instruction: Below is the grading scale which will be used in rating your performance.
5 - Excellent - 95-100
4 - Very Satisfactory - 88-94
3 - Satisfactory - 82-87
2 - Fair - 76-81
1 - Needs Practice - 70-75

Definition: Anthropometric measurements and weighing are used to assess newborn size
against standards and identify nutritional risk.

Materials:
 Tape measure
 Weighing scale
 Drop light
 Baby wrap
 Surgical gloves

Procedure:
Preparation: 5 4 3 2 1 Remarks
1. Prepare articles completely.

3. Provide for privacy.
4. Perform hand hygiene, and observe other
appropriate infection control procedures.
5. Place the newborn on a supine position.
6. Check that the scale is on a flat, level
surface, with no obstructions.
7. With the scale empty, ensure that the 0
figure appears on display.
Procedure:
Weighing
8. The baby should be naked before placed
on the scale. Remove any clothing worn by
the baby. An incandescent light bulb can be
positioned over the scale to warm the surface

of the scale, making it more comfortable for


the baby.
9. Carefully place the baby on the scale while
supporting the occipital area and the
buttocks. Wait for the baby to settle, until a
stable weight is measured.
10. When the display freezes, read and record
the weight.
11. Carefully remove the baby from the scale
and place back into the examination table /
isolette / bassinet.
Head Circumference
12. With the infant lying on a supine
position, loop the tape before slipping it over
the head. Take care that the side of the tape
marked in centimeters is on the outside for
the reading, with the zero end in the inferior
position.
13. Anchor the tape just above the eyebrows.
At the back of the head, the tape is positioned
over the fullest protuberance of the skull.
14. Once the tape is positioned correctly, pull
tight to compress the hair and subcutaneous
fat. Be careful not to pull the tape too tight
and cause injury to newborns. Keep hands
and fingers out of the way for the reading.
15. Read and record the measurement and
carefully lower the tape from the infant’s
head to his chest.
Chest Circumference
16. With the infant still lying in the supine
position, bring down the tape measure behind
the infant’s back.
17. Run the tape measure across the infant’s
chest. It should be along the nipple line.
18. Read and record the measurement and
carefully lower the tape from the infant’s
chest to its abdomen.
Abdominal Circumference
19. With the infant still lying in the supine
position, bring down the tape measure behind
the infant’s waistline.
20. Run the tape measure across the infant’s
abdominal area slightly above the umbilical

area.
21. Read and record the measurement and
carefully remove the tape from the infant’s
abdomen.
Mid-arm Circumference
22. With the infant still lying in the supine
position, place the tape measure to the
infant’s arm.
23. Run the tape measure across the infant’s
middle arm. It should be in between the
shoulders and the elbow.
24. Read and record the measurement and
carefully remove the tape from the infant’s
arm.
Length
25. Position the infant in a lateral position.
26. Place the tape measure starting from the
back of the infant’s head following the
contours of the body.
27. Run the tape measure along the infant’s
spine. Press against the buttocks and
popliteal area.
28. Read and record the measurement and
carefully remove the tape from the infant.
29. Cover the baby with a baby wrap and
place under a droplight to aid in
thermoregulation.
30. Perform after care of articles used.
31. Observe appropriate infection control
procedures, and perform hand hygiene.
32. Record findings.
