PES Worksheet 25 points

Evaluate the following PES Statements. Identify which part(s) are incorrect and identify why.
1. Inadequate oral intake related to decreased appetite as evidenced by exercise routine.
_____“Exercise routine” would not be considered a nutrition sign/symptom measurement. For instance,
it should include a numerical value that can be resolved and supports the root cause such as
“consumption of <50% of estimated energy requirements” or “eating x% of usual intake”.______

2. Food and nutrition knowledge deficit r/t eating 3 cookies at lunch aeb 24 hour recall.
_____In my opinion, each part of this PES statement could be altered being that it doesn’t entirely make
sense and/or flow together. For example, a 24 hour recall does not necessarily mean that the client is
unknowledgeable in regard to food and nutrition. Instead the etiology should insist no previous nutrition
education. As an alternative, I would use “Excessive energy intake r/t undesirable food choices aeb 24
hour recall revealing 3 cookies consumed at lunch” or “undesirable food choices r/t food nutrition
knowledge deficit aeb 24 hour recall.______

3. Excessive weight gain r/t eating fruits and vegetables aeb 24 hour recall.
_____Eating fruits and vegetables would not necessarily cause excessive weight gain. Therefore, the
root cause does not make sense. For instance, excessive weight gain may be caused by consistent
consumption of fast food, which would then be documented in a 24 hour recall. However, it would be
better to address a numerical value for the sign/symptoms such as BMI or X lb. weight gain over the last
X amount of months._____

4. Micronutrient inefficiency r/t eating too much protein aeb 3 pieces of chicken per day
_____This PES statement does not make sense. Protein is a macronutrient, yet this statement is
addressing a micronutrient inefficiency. If a micronutrient inefficiency is the problem, then the PES
statement should specify the appropriate etiology and possible laboratory values. Though, in the case of
eating too much protein, this statement could benefit from changing the problem to something along the
lines of “Excessive oral intake” or “Excessive protein intake” r/t a certain belief or knowledge deficit
aeb “exceeding protein requirement by X%”.

5. Not eating enough calories r/t 24 hour recall meeting recommendations aeb 24 hour recall.
_____This PES statement also does not make sense. If the client is not eating enough calories than they
most likely would not be meeting recommendations. Rather, the statement could read “Inadequate
energy intake r/t decreased appetite or nutrition status/new diagnosis aeb 24 hour recall or meeting X%
of energy requirements or recent weight loss of x lb. over x period of time”. ____

1. 20 yo F – eats the recommended amount of all food besides fruit because she was told it has too much
sugar.
____Inadequate intake of fruit r/t food and nutrition knowledge deficit aeb belief that fruit has too much
sugar and doesn’t meet recommended values.___

2. 18 yo M – intake does not meet required recommendations because he is too busy to eat
____Inadequate energy intake r/t perception that limited time prevents selection/food choices consistent
with recommendations aeb not meeting required recommendations.____
3. 25 yo M - only drinks protein shakes because he wants to gain muscle mass, is overweight by 30 pounds
and does not eat vegetables.
___Overweight r/t food and nutrition related knowledge deficit aeb does not eat vegetables, drinks only
protein shakes and is 30 lbs. overweight.___

4. 82 yo M – has lost 10 pounds recently because he lost his appetite


___Inadequate protein-energy intake r/t loss of appetite aeb recent 10 lb. weight loss.___

5. 56 yo F – struggles with trying to lose weight, is 35 pounds above her IBW, and is eating 3000 kcal per
day.
___Overweight/obesity r/t excessive oral intake aeb 35 lb. above IBW and energy intake of 3000
kcals/day.___
