4. HW Description
The myQA Daily device is a device for daily quality assurance (QA) measurements in radiation therapy.
The detectors are ionization chambers arranged in a special pattern for obtaining accurate results in a
most efficient way. With the myQA Daily SW, daily QA and trend analysis of dose output, flatness,
symmetry, center, field size, and energy are finished in one measurement.

Electronics

Fig. 4.1. myQA Daily (drawing) - top side and left side. The printed markers are:

 X and Y-axes and two diagonal axes. The point of intersection is the iso-center
 Locations of part of chambers (on the X and Y-axes and the center area) are indicated with the “+”
sign.
 Printed field size marks, “10 cm × 10 cm” and “20 cm × 20 cm”, are scaled on the surface to take
into account of the projection from the source to the surface.
When the myQA Daily device is set up at isocenter SSD = 100 cm (aligned to the markers on the
sides), on the device surface:
“10 cm × 10 cm” is 9.93 cm × 9.93 cm
“20 cm × 20 cm” is 19.88 cm × 19.88 cm
 Alignment markers on the sides (7 mm below the surface)

4.1. 2D Detector Array

The myQA Daily has an array comprised of 125 vented ionization chambers, distributed in a 5 mm grid
over a 25 cm × 25 cm area. The ionization chambers are individually connected to high resolution
current-to-digital converters, measurement values are digitally transferred to the myQA Daily application
servers via integrated wired and wireless networking interfaces.
Chamber placement has been optimized to suit the measurement of 10 cm × 10 cm and 20 cm × 20 cm
radiation fields and groups of ionization chambers are used for individual measurements:
1. High-resolution centerline for field size and penumbra measurement
28 ionization chambers for each centerline (Fig. 4.2, 1). The chambers are higher resolution in
vicinity of standard 20 cm × 20 cm or smaller 10 cm × 10 cm penumbra regions to achieve higher
measurement accuracy for the field size and penumbra measurements of standard 20 cm × 20 cm
or smaller 10 cm × 10 cm beams.
2. High-resolution center-area for output measurement (Fig. 4.2, 2)

3. Energy constancy checks
Four dedicated ionization chambers with integrated absorber material (Fig. 4.2, 3) for automatic
verification of photon / electron energy constancy. The materials and thickness of the absorber are
listed the Section 9.3.
For energy constancy tests, the SW calculates an energy factor as the ratio of two energy
chambers. The two energy chambers are predefined such that said energy factor is most sensitive
to slight spectral changes of the selected beam quality. Typically, a change of 1% of the energy
factor corresponds to less than 1 mm shift of the corresponding PDD in water.

Beam Symmetry

Beam symmetry Sx and Sy is calculated from the uniformity- and kTP -corrected measurement signals
Mi,j of the chambers marked in Fig 4.3 for the two reference fields. Indices i and j denote the chamber
position in X and Y direction, with i = j = 0 at the center position.
For crossline symmetry, the ratio RX,i,j = Mi,j / M−i,j is calculated from the measurement signals of each
chamber and its mirrored counterpart. The maximum ratio in percent is the measurement value for field
symmetry in crossline direction: SX = 100% · max{ RX,i,j }. Similarly, the field-symmetry in inline direction
SY = 100% · max{ RY,i,j }, where RY,i,j = Mi,j / Mi,−j.

Beam Flatness

Beam flatness F is calculated from the uniformity- and kTP -corrected measurement signals Mi,j of the
chambers marked in Fig 4.3 for the two reference fields. Indices i and j denote the chamber position in
X and Y direction, with i = j = 0 at the center position.
F = 100.0% ▪ max{ Mi,j } / min{ Mi,j }

Note: Beam flatness is also calculated for FFF beams. In the case of FFF beams, it is normal to obtain
unusually large values for the flatness parameters that also may be very sensitive to variations in
beam center offsets. If this raises a problem, please increase the acceptance tolerance for the
flatness parameter in the test-setup.

5.4. Preparing myQA Daily Device for Measurements
The detectors of the myQA Daily device are ion chambers, a certain dose of pre-irradiation before the
chamber response reaches a stable level is needed. Background level is measured and compensated via
software automatically.

5.4.1. Setting up the Device under a LINAC

Place the device on the treatment couch under the treatment head such that the device electronics
faces away from the LINAC gun and is not within the irradiated area (Fig 5.12).

myQA Daily User’s Guide | Setting up for Measurements | 27

5.4.2. Device Warm-up and Pre-Irradiation
Warm-up

The myQA Daily device does not need warm-up to reach its specified measurement accuracy.

Pre-irradiation

Ionization chambers need a minimal amount of pre-irradiation to reach a stable response and their
maximum measurement accuracy.
When using the myQA Daily device with radiation fields of less than 100 MU and less than 1 Gy dose to
the ionization chamber array, a minimum amount of pre-irradiation of 0.25 Gy (field size: 25 cm × 25 cm).
If the dose of the irradiation is higher than or equal to 100 MU and the dose to the ionization chamber array
is higher than or equal to 1 Gy, no pre-irradiation is necessary.
For simplifying the procedure, it is recommended to pre-irradiate with 500 MU during the LINAC's warm up
phase in the morning.

5.4.3. Background Measurement

The device automatically performs background measurements every 20 seconds to compensate for
ionization chamber leakage while no irradiation is taking place.

5.5. Shutdown Procedure

Ensure that no active task is being run in the myQA Daily web application.
Press the power button on the myQA Daily device to turn off the power.
If powered by an external power supply, disconnect the external power supply once the LED ring
surrounding the button is no longer lit.
Log out myQA Daily application.

7. SW Application 2 - Operation
7.1. Test Run Page
After a QA task has been set up on the Setup page (Section 6.7.2), it can be executed on the Test Run
page. All tasks set up in a week are displayed on the Test Run – Task Overview page (Section 7.1.1)

About running a task

■ When one user starts executing a task for a specific machine, no other user can access that task. Only
after finishing the task, it can be viewed by other users.
■ If a task is in execution, it is displayed on the Test Run - Task Execution page (Section 7.1.2).
■ Task results are displayed only for the current day on the Test Run – Task Execution - Summary
page (Section 7.1.2.4) and Test Run - Summary page (Section 7.1.3). The results of previously
executed tasks can be viewed only in the Trends page (Section 0).
■ Task executions can be repeated.

Measurements of a sequence of beams or single beam

There are two ways to execute a task run: (1) measuring a sequence of beams (Section 7.1.2.1) - that
expects all beams to be executed in the defined order and (2) measuring individual beams (Section
7.1.2.2).
When creating a task, a physicist can define multiple beams for a task in the order that they are
programmed in the LINAC so that the measurement sequence (beam order) matches that in the LINAC
program. In this way, the user interaction in myQA Daily is minimized when the task is performed as a
sequence for all beams.
If single measurements already exist when a measurement sequence is started, all beams will be re-
measured. A measurement sequence can only be executed for all defined beams. There is no partial
execution.

Halt a task execution

It is possible to halt a task execution and continue the same task execution within two hours. After two
hours, any measurement data will be discarded, and the task will show as not in execution. This means, in
case of a network interruption or a browser crash, an existing task execution can be resumed just by
opening the task execution again. No data will be lost if it happens within two hours of the start.
If a task execution has been started and the user reopens it, a dialog with the option to resume (keep
already done measurements), restart or discard is displayed.
Sequence measurements can be stopped. A dialog asks the user if measured results should be retained or
discarded.

Note: When the device is disconnecting, after measurement is aborted, the browser might not respond.
Please refresh the browser by pressing F5 key on the keyboard to return to the test execution page.

7.1.1. Test Run – Start-up Page: Task Overview

After entering the login credentials, by default, a start-up page will open the Test Run – Task Overview
page on the current day of the week of all machines (Fig. 7.1) if daily tasks have been set up for at least
one machine and reference measurements are available (Section 6.7.2).

42 | SW Application 2 - Operation | myQA Daily User’s Guide

If a machine is bookmarked to, e.g., a certain machine (Section 6.8.5) and the application is opened with
the bookmark, the start-up page is the Test Run page on the current weekday of the bookmarked
machine. The daily QA check procedure can be started directly with this machine (Section 8.3).

All Machines or an individual machine, the corresponding tasks of selected machine(s) are displayed
in blocks.
2. Execute date filter – it includes all days and all days in a week. All tasks in a week (by selecting All) or
the tasks of a particular day (by selecting this day) are displayed. By default, the current weekday is
selected. However, a different day can be selected so that the tasks not initially planned for the current
day can be executed.
When tasks are executed, they are always executed for the actual current day. The execution day
cannot be modified to an earlier or later day, i.e., the task results show up on the current executing day
but not its initially planned day. On the initially planned day, this task is still listed as not executed.
3. Main display (task blocks): displays the overview of tasks for the selected machine on the current day
(by default, or a user-selected day) in blocks. If a weekday is selected: The tasks are sorted by status
first, then machine name. If ‘All’ days are selected: The tasks are sorted by machine name (status is
ignored).

8. Typical Application Workflows
In this chapter, general workflows for QA daily checks with the myQA Daily solution (HW and SW) are
described.

8.1. Equipment Preparation

1. Set up the myQA Daily device under a LINAC (see Section 5.4.1 for details)
 Place the device on the treatment couch under the treatment head such that the device electronics
faces away from the LINAC gun and is not within the irradiated area.
 At the LINAC console, choose the first field to be irradiated; turn on the light field.
 Align the side makers on the device to the room lasers to position the device in the center of the
beam, check congruence of lasers and light-field projection.
2. Turn on the myQA Daily device
3. If the dose of the first QA field to the detector array < 1 Gy, perform a pre-irradiation (0.25 Gy, 25 cm ×
25 cm)

8.2. Procedure for Setting up a QA Task

See Section 6.7.2.

8.3. Executing QA Tests with SW Application

Note: For detailed instruction, please refer to Section 7.1 and Section 0.

This a routine procedure for repeating the daily tests under the conditions that the measurement setup
(HW and SW) has been completely done.
1. Log in and go to the Test Run page of the machine to be checked. Today’s tasks will be displayed
2. Click the Start button in the task block which task will be executed.
3. If a security checklist exists, go through all check-items and check the corresponding box if it is passed.
4. Click the START MEASUREMENT SEQUENCE button and than turn on the beam to start the first
beam measurement.
5. Follow the instruction in the Measurement Sequence dialog to finish the measurements for the rest of
beams.
6. 3. Finish task by clicking the FINISH button.
7. For users of the role physicist, a task result can be overwritten by clicking Overwrite Status. An
overwrite reason is required to fill into the box below.
8. Check trends
Go to the Trends page to view trends for selected parameters and export trend charts.

myQA Daily User’s Guide | Typical Application Workflows | 71

8.4. Recommendations
■ If possible, choose a field size of 20 cm × 20 cm. Use 10 cm ×10 cm only if the accelerator is not able to
provide the larger field-size.
The larger field size allows for better measurements of beam flatness and symmetry using a larger
number of detectors.
■ Choose a different number of MUs for different fields.
Choosing different numbers of MU makes it easier to identify accidental miss-irradiation or out-of-order
irradiations of fields.
■ The first field in your irradiation sequence should have ≥ 200 MU to minimize the effect of device warm-
up and to make pre-irradiation unnecessary.
Ionization chambers generally require pre-irradiation to obtain stable readings. However, the required
ionization chamber dose was observed to be very small (0.25 Gy) and the introduced inaccuracy by not
pre-irradiating the chambers is smaller than ±0.5 % for a total irradiation dose of 1Gy or larger.
