    2. Basic Letter Request Address to Chief PNP (Attn: D, PRBS)

    2. Updated Service Record
    3. Commutation Order
    4. Breakdown of Leave Credits
    5. Latest Promotion Order (For NUP: Salary Adjustment)
    6. Non-money/ Property Clearance
    7. Certificate of Last Payment
    8. SALN
    9. DC Clearance
    10. DL Clearance
    11. Authorization (Affidavit) to Deduct all Financial Obligation
    12. UPDATE FORM/ STI1/ ATM Account number
    13. Contact number (Cellphone number)
    14. 2×2 Picture with Nametag
    15. Pensioner’s Information Form
    16. Family Information Sheet/Retirees beneficiary/ies (please Fill Up)Duly Notarized)
    Additional Requirements for Deceased PNP Personnel
    1. Decree of Entitlement
    2. Certificate of Legal beneficiaries
    3. Death Certificate (From PSA with OR from LCR if not yet more than 6 months)
    4. Marriage Contract (From PSA with OR)
    5. Marriage Contract of Parents (From PSA with OR, if Single)
    6. Advisory on Marriage (husband/ wife, from PSA with OR)
    7. Birth Certificate of all children (from PSA with OR)
    8. Birth Certificate (Husband/ wife, from PSA with OR)
    9. Certificate of no Marriage (CENOMAR, if SINGLE)
    10. 2×2 Latest ID Picture of PNP Personnel

    5. Directorate for Logistics (DL) Clearance
    6. DLOD Clearance
    7. Ombudsman Clearance
    8. NAPOLCOM Clearance
    9. Certificate of Last Payment (CLP)
    10. Statement of Assets, Liabilities & Net Worth (SALN) – Original
    11. 2×2 Picture with name-tag
    12. STI-1 Pensioner Account Information with Bank Confirmation Slip
    13. Additional Form Retirees Beneficiary (Fill-up)
    14. Cellphone Number
    15. Update Form
    16. Pre- Retirement Seminar (Online)
    17. Authorization to Deduct
    Additional Requirement for Outright Pension:

    Affidavit stating that he/she is waiving his/her right for payment of the three (3) Years
    Lump Sum and opting to be paid through Outright Pension.

    Additional Requirement for Outright Pension with Pending Case:
    – Affidavit of Undertaking
    – Declaration of Pendency/ Non Pendency of Case
    – Affidavit of Authorization to Deduct

    TRANSFER OF PENSION

    Transfer of Pension (1 folder with tabbings PSA original documents):
    1. Affidavit of Authenticity on the submitted documents (notarized)
    2. PSA Death Certificate with Official Receipt
    3. Advisory on Marriage of Retiree (PSA with Official Receipt)
    4. Advisory on Marriage of Wife with OR (PSA with Official Receipt)
    5. 2×2 latest picture of PNP retiree
    6. 2×2 latest picture of wife
    7. PSA Marriage contract with official receipt
    8. PSA Birth Certificate of Wife with Official receipt
    9. PSA Birth Certificate of Children Under (18) Eighteen years of age witan h official receipt
    and 2×2 latest Picture of Children
    10. Photocopy of 2 Valid Identification Card of Wife with Signature, Thumbmark and
    Cellphone Number
    11. Affidavit of Authenticity to deduct for back earned
    12. Pensioner’s Information Form

    Other Requirements for Inclusion of Minor Children:


    1. Affidavit of Guardianship
    2. Approved CLB/ Decree of Entitlement
    3. PSA Birth Certificate
    4. 2×2 Picture of Minor Child/Children
    5. Photocopy of IDs with Cellphone Number
    6. Pensioner’s Information Form

    1. Medical Abstract of PWD
    2. Approved CLB/ Decree of Entitlement
