The undersigned 4962 OFFICER C. SULLIVAN - Unified Police Department, Agency Case
No. CO22-85937, upon a written declaration states on information and belief that the defendant,
JEFFREY KEVIN LOOSLE, committed the crime(s) of:
COUNT 1
SEXUAL BATTERY, 76-9-702.1, a Class A Misdemeanor, as follows: That on or about May 4,
2022 in Salt Lake County, the defendant did under circumstances not amounting to rape, rape of a
child, object rape, object rape of a child, forcible sodomy, sodomy on a child, forcible sexual abuse,
sexual abuse of a child, aggravated sexual abuse of a child, aggravated sexual assault, oe an attempt
to commit an of these offenses, intentionally touched, whether or not through clothing, the anus,
buttocks, or any part of the genitals of another person, or the breast of a female person, and the
actor's conduct was under circumstances the actor knew or should have known would likely cause
affront or alarm to the person touched.

Your affiant bases probable cause on information and evidence collected by Unified Police
Department, Case No. CO22-85937 and the following:
The statement of . that on or about May 4, 2022, she was attending a training conducted
by her employer in Salt Lake County. . stated she was standing near a table when she felt a hard
slap on her butt, which jolted her body. turned around and saw her co-worker, JEFFREY
KEVIN LOOSLE, sitting behind her and making direct eye contact with her. . stated no one else
was in the area. . stated, “What the ?” and LOOSLE did not respond. . stated she was
again slapped on her butt with an unknown object. . stated she confronted LOOSLE the next day
with another colleague. . stated LOOSLE did not deny slapping her butt but stated that he needed
to get her attention. LOOSLE then stated he didn’t slap her, he hit her with her coffee cup.
LOOSLE asked . if she had told her husband.
The statement of ., ,’s husband, that approximately a week after the incident,
LOOSLE reached out to him and apologized. LOOSLE told . he thought . was “one of the
guys” and admitted to smacking her with a boxing glove.

The statement of Detective C. Sullivan with the Unified Police Department that he made
contact with LOOSLE. LOOSLE stated he was sitting near the table alone, and . was standing on
his right, engaged in a conversation with an unknown party. LOOSLE stated he wanted to add his
opinion to the conversation, so he used a boxing glove to touch . to get her attention. LOOSLE
stated he attempted contact with .’s arm, but the boxing glove “went limp,” and he was unsure
where it touched her. LOOSLE stated a week after the incident, he reached out to .’s husband to
“tell him his side of the events.”
