Hypertension

The student nurse created a care plan for a 72-year old male patient admitted for weakness and fatigue due to uncontrolled hypertension. The plan included assessing factors contributing to fatigue, observing the patient during activities, and teaching energy conservation techniques. The goals were for the patient to independently perform daily tasks, participate in self-care, and demonstrate decreased physiological signs of intolerance. The nursing interventions and evaluations aimed to reduce fatigue and help the patient sustain an optimum level of functioning safely.
