(a). Documents to be filled against previous installment you have received:
1. Progress Report
You must fill it for all the previous semesters duly signed and stamped by Head of Institution of University/Institute.
2. Scholarship Fund Utilization Report of Tuition Fee
You must fill it for Previous installment received and submit duly signed and stamped by Head of Finance Division,
Internal Auditor & Head of Institution of concerned University/Institute.
Note: You must fill it against amount you have received in last installment only as Tuition fee/hostel Fee/Other Charges
and please avoid to mention stipend amount on this Performa.
3. Acknowledgment Receipt of Stipend
Must be filled against stipend amount you have received in last installment and please avoid mentioning tuition fee
amount on this Performa.
(b). Documents to be filled for next payment:
4. Invoice Performa
For next Semester/Year will be signed and stamped by Head of Finance Division of University/Institute.
Invoice Performa must be filled/attested (signed & stamped) by Respective University/College Finance/accounts Section.
Please mention only those non-refundable charges in invoice Performa which you have paid or will pay to your
university, If you have completed more than one semester send separate invoice for each semester.
5. Verified Result cards of all previous semesters/ years.
Photocopy of Degree (if you have completed your program)
6. Fee Structure of Complete Program
Fee structure of complete program on College/University Letter Head and must be verified (Singed & Stamped) by
Director Finance/Treasurer /Account officer.
7. Fee slips/ Bank Challan
Bank Deposit Slips, Challans/Fee Slips (Scholar must attach copies of bank challans with the Invoice Performa.
(Charges paid to University & charges mentioned in invoice must be same).
8. CNIC copy of Scholar and father & 02 Passportsize Photographs

Cutting and overwriting on any document is not allowed.
