Reason for Referral

A referral has been offered to the client to evaluate and determine her current personality situation owing to
her past traumas that caused her severe distress. This is to find out what was the effect of these past experiences to
her and what will be the pace to establish for her to overcome it gradually. One of the main reasons for a referral is
she a victim of abuse by her ex-boyfriend verbally and physically questioning herself to why it did happen. Moreover,
due to her past experiences there are many sources of stress leads her to think of a self-harm and unable to control
of the things that is uncontrollable that makes her overthink a lot.

As part of referral, a consent was formulated for the assessee indicating that she will be referred for a
counseling and evaluation to understand herself more. The client is willing to coordinate with this referral as she also
wanted to have a counseling session to better know herself and how she could overcome it step-by-step.

Background Information

The assessee, Jeorgia Ejes, is a 22-year-old currently studying on a Second-year college level. She is
currently living in Cainta, City of Rizal. She is a working student at the same time and started to provide her personal
needs as well as providing her family’s necessities as a Call center agent. She wanted to be a Flight Attendant but
also wants to be a Lawyer and a Doctor. A family-oriented person raised in a typical and usual family tradition who’s
been eyeing her to graduate in college. A person described as a social butterfly due to her active social interactions.
Both extrovert and ambivert. A typical student who considers quitting studying due to unable to catch up from the
online school. The assessee considers herself being independent, fearless, brave, and resourceful as her strengths.
While her weaknesses appear every time that she is overpowered by her emotions. What’s most important to her is
to feel fulfilled that she be driven to have a passion for what she does. There are main sources of stress the
assessee experienced, financial matters, time management, work, and academics and uses her coping mechanisms
to swerve the struggles. Always been eyeing her friends and family as her support system during her hard times.

The assessee has a history of abuse. She was abused by her ex-boyfriend through verbal, physical,
emotional, mental, and sexual abuse as well as violence. A medical history that she was diagnosed a Diabetes as
her blood sugar is not normal at her age. These traumas caused her traumas and severe distress leading her to self-
harm.

Behavioral Observations

During an interview with the assessee telling her side of the story, it was observed that she is anxious about
herself. She was severely affected by her past traumas until now while sharing her stories. A person who had been
going to through and unable to manage her time due to a self-pressure and pressure that she is experiencing from
her family. I can claim that she is fragile due to the experiences and past traumas that she has been handling
wherein she is still unhealed from. As I pertain to fragile, she is not weak as what I carefully observed because she
is strong enough to open to other people that she is not close with the dark moments of her lives with lightness of
conversation. At the end of the session, the assessee tells me at the end of the session that the entire interview has
eased of her burdens. The assessee was not hesitant to open during the interview.

Test Results

The tests that were used to administer are Bender Gestalt, Sacks Sentence Completion Test (SSCT), Work
Values Inventory (WVI), Neo pi-r, and Beck’s Depression Inventory. The assessee resulted a classification between
high scores. For copy phase, she got a standard score of 113 in a percentile rank of 80.69% as the high average.
While on the recall phase, the assessee got a high classification which shows that she can recall visual
representations and has a capacity to recall even the hardest ones. The SSCT has four areas wherein these areas
are divided for different values. The SSCT were scored based on three (3) scale value 0 for not disturbed, 1 for mildly
disturbed, and 2 for severely disturbed. Most of the values obtained 2 score which shows that she is severely
disturbed by her personality and behavior than her family and friends. As for the Work Values Inventory (WVI), the
assesee obtained fifteen (15) to Creativity, Achievement, Way of life, and Intellectual Stimulation meaning Jeorgia
aims to maintain this level of behavior to fulfill her objective as a worker or employee and be satisfied with her job.

A. Bender Gestalt Test

The Bender Visual-Motor Gestalt Test is a screening tool that may be used to identify or assess brain
damage, visual-motor maturity, visual-perceptual skills, and emotional disorders in both children and adults. It also
gives an assessment of how they responded in style. It is not recommended for anyone who has a serious vision
impairment or severe motor impairment.

Based on the particular result that was gathered, Jeorgia Ejes has come up with her capacity to be flexible
and certain to every situation or event that she might put into. She can use the English language as her common
medium appropriately and she is aware of grammar usage and vocabularies. Well-founded reading comprehension
and communication skills particularly on both verbal and non-verbal. These abilities can absolutely set as her bread

The overall summary findings reflect how each specific topic was rated by the interviewer. The items
concerning (1) Attitude towards her father, which mentioned their distance with each other, had the highest score
that resulted in an interviewee that significantly upset or distressed her. Her father, according to the assessee, is
impatient and gets tired of supplying for his family's needs. An observation of their aloofness made her think of her
father's negative behavior. She is severely disturbed by her concerns of being left out and losing those who are close
to her, as seen by her Attitude towards Fears (2) rating. During an interview, she also stated that one of her circles of
friends had been estranged from her due to her job and that she was unable to communicate with them. This
portrays her as a sociable and friendly individual with whom you can get along. I, the assessor, have seen that her
attitude regarding her ability is the source of her severe distress (3). She is frequently demotivated and easily

influenced by circumstances. When things do not go as planned, she instantly doubts her ability and becomes
engaged in negative thoughts. While on her attitude toward guilt feelings (4), she wanted to forget the time that she
had to deal with the people who treated her less of a person and getting herself lost when everything is out of her
control. Considering this, the interviewee shows that she can still manage her emotions when things do not go as
planned throughout our interview. She expressed her brave spirit by striving to be better and to overcome her
difficulties. Her attitude toward her future and ambitions (5) reveals that she is a hopeful, goal-oriented teenager who
wants to see her dreams fulfilled. She was aware of her chances of achieving her goals and was aware of where her
heart belonged. She is said to be a firm belief in herself.

C. Work Values Inventory

The WVI was created in response to a need for a method of evaluating the goals that inspire people to work.
It is intended to quantify both extrinsic and intrinsic values in work, as well as the satisfactions that men and women
seek in work and the satisfactions that may be key components or results of work. The test consisted of fifteen (15)
values which are altruism, esthetic, creativity, intellectual stimulation, achievement, independence, prestige,
management, economic returns, security, surroundings, supervisory relations, associates, way of life, and variety.

The interviewee named Jeorgia Ejes obtained a high score of fifteen in four (4) different values which are
Creativity, Achievement, Way of Life, and Intellectual Stimulation. Given these values, Jeorgia aims to maintain this
level of behavior to fulfill her objective as a worker or employee and be satisfied with her job. As a result, she most
likely prefers to work in an atmosphere that she finds appealing. She also prefers a working atmosphere in which she
may form positive relationships with her coworkers, develop new ideas, and experience a sense of success from her
work. The assessee is a positive and capable individual who makes her own decisions about what is best for her and
wants to make her own decisions. She obtained fourteen (14) in the values Su (Surroundings) and Va (Variety). It
implies she desired a work atmosphere that would allow her to continue to enjoy her career. She desired a
workspace that was comfortable, quiet, and well-organized. Furthermore, having a job that will give many
opportunities to excel and take you to your comfort zone, bringing out your finest qualities for the work, is essential
for her. Es (Esthetics), Er (Economic Return), and Al (Altruism) got a score of thirteen (13). Because of this, Jeorgia
is likely to meet her standard. A location where she may participate and be a part of creating beautiful things.
Leading and having a fulfilling life while working is as essential to her as getting paid enough to live comfortably and
meet her necessities. She longed to assist others, driving her to seek employment in the field of social welfare. The

interviewee's lowest score was Ma (Management), which indicates that she is unable to manage others or use her
leadership qualities because she is apprehensive of leading the team or group.

D. Neo pi-r

This is to assess the main strengths and weaknesses of the assesee. There are personality levels of the 5
dimensions of the personality. This is also a most widely known and used assessments by the professionals in
administering and evaluating the client.

The score that was obtained to this aspect is high which indicates that assessee is easily upset, even by
what most people consider the normal demands of living. People consider her to be sensitive and emotional.

Summary Result

Integrate the results of all the test here and the interview. Hypothesis building and thorough discussion of the

if there is a disorder present.
