Case Application:

Kuya Rene, a 48-year -old bus driver suffered a vehicular accident. He was rushed to the
hospital and was given emergency treatment for severe blood loss. Rene's lower left leg was torn
off upon impact. Upon examination, it was revealed that he suffered complete fracture of the leg,
severe injury to the right inner thigh and foot, massive muscle damage and head injuries.
Emergency surgery was initiated to repair extensive damage. However, after surgery Rene went
into coma (state of unconsciousness) due to a developed blood clot on the brain. He underwent
another operation to remove the clot, and it was very successful. He was in comatose state for
several days and eventually woke up but he became paralyzed. The nurses cared and cleaned his
body, do bedside exercises, etc...

Because of his will and faith, after a few months, Rene was able to sit in bed and could move
his legs little by little until after a few weeks he could stand and walk with a limp. The physician
ordered that the patient be given and taught the use of crutches as assistive device. Lissa, an
orthopedic nurse, was tasked to perform health teaching and guidance to Kuya Rene.

1. Cite a situation in the case where the nurse can employ or apply the three (3) support system
modalities - wholly compensatory system, partially compensatory system and supportive-
educative system.

Wholly compensatory system

 Kuya Rene suffered from many serious injuries and brain traumas where he performed
several emergency surgery that resulted into coma and he have been unconciousness and
later on he became paralyzed. Whole compensatory must be provided to the situation of
Kuya Rene where he will depend to his nurse to do all the therapeutic self-care including
the daily body maintenance, feeding, bathing and positioning his body. Since, Kuya Rene
can not able to participate and provide for his fundamental needs. Full self-care is what
will the nurses do for her whole being.

Partially compensatory system

 Kuya Rene was given a crutches for temporary support that to be teach by his nurse. He
can able to utilize this assistive device through instructing how to properly use them.
Nurse must evaluate the safety measures and restrictios when using these crutches. Kuya
Rene can not able to utilize the crutches properly if he does not have an assistance in
using it and also a physical support must be instilled in getting up or laying down on bed.
The nurse and Kuya Rene would team up for better and effective utilization of assistive
device.
Supportive-educative system

 Ms. Lisa as an orthopedic nurse was there to educate Kuya Rene in his conditions and
complication for him to be able to fully recover from his fractures or injuries. Ms. Lisa
can assess health prevention on how could he properly take care of his healing injuries.
As Kuya Rene was still in the process of healing, he must need a nurse for him to get a
health informations and to maybe have monthly monitoring if he needs a follow up care
if necessary, in that way he can now be wholesome being with no complications.
