Dear Healthcare Partners,
The state supply of various medical materiel has been available to help support state agencies, counties,
tribes and other partners and organizations through the COVID-19 pandemic. This resource will only
continue to be available to support organizations in extraordinary circumstances such as when supplies
are unavailable through regular supply chains, when shipping delays cause an immediate need, and
when urgent needs arise that cannot be addressed through other means.
The medical equipment contained within this catalogue was either purchased by Washington State or
issued from the Federal Government, in order to assist Local Health Jurisdictions (LHJ’s) and Private
Healthcare Organizations (PHO’s) in their response to the Covid-19 pandemic. The Federal Strategic
National Stockpile ventilators are on loan for a to-be-determined time. The State procured medical
equipment is provided free of charge and its ownership can be transferred, if desired.
NOTE: The Washington State Attorney General has determined that, in a time of public need, state
procured assets can be gifted to LHJ’s and PHO’s. If you have any questions concerning this subject,
please contact the Washington State Medical Surge Logistics Chief at the e-mail below.
If you would like more information about this equipment, the quantity available, or technical changes
to this catalogue, please contact the WA State Medical Surge Logistics Chief.

Placing a Request
PPE Requests
The state PPE supply will only continue to be available to support organizations in extraordinary
circumstances such as when supplies are unavailable through regular supply chains, when shipping
delays cause an insufficient level of PPE, and when urgent needs arise that cannot be addressed
through other means. If a need is accessed pleas go through the below request steps.
1. If an urgent need exists, contact your county emergency management agency to determine
how orders should be placed with them. They are able to answer questions about the forms and
ordering process.
2. Complete the forms/process provided from county emergency management.
3. If local emergency management is able to fill this request from a locally existing supply, they will
let you know how that process will be completed.
4. If local emergency management is not able to fill the request, and determine the need is urgent
they will send the request to the state for processing.
State agencies, federally recognized tribes, and local emergency management agencies are eligible
