Perineal Cleansing Before Delivery

•The perineal region should be thoroughly cleansed prior
to delivery. This procedure is called “sterile prep.”
•The sterile prep is done primarily to prevent infection
and secondarily to increase the visibility of the area by
removing the bloody discharge that is present prior to
delivery. Needless to say, providing thorough perineal
cleansing promotes comfort.
•The sterile prep can be carried out in different ways
depending on the institutional policies. In some
hospital, sterile gloves are used while in others, the
use of sponge sticks/holders is sufficient. But the
principles are the same: the sterile prep is done to
help prevent infection and care should be observed
during the sterile prep so that the birth canal is not
contaminated.
•With the woman positioned
for delivery the health
care provider proceeds to
cleansing the vulvar and
perineal areas observing
strict aseptic techniques.
PERINEAL STERILE PREP

• Wash hands thoroughly.


• Depending on the hospital policies,
wear sterile gloves or use sponge
holder.
• Using a cleansing solution and the
first sterile sponge, begin
cleansing from side to side
beginning from the mons
veneris/symphysis pubis up to the
umbilicus. Discard sponge.
•With the second and third sponges, clean the area from
the labia to midthigh of each leg using up-and-down
motion using one sponge per leg. Each sponge is
discarded after use.
•The movement of sponge is from inward (vulvar region)
to outward (thigh region) to prevent soil carrying
soil/discharge from surrounding areas to the vaginal
outlet.
•The fourth and fifth sponges are used to cleanse the
labia (left and right) with one downward sweep avoiding
the rectum.
•The sixth sponge is used to cleanse the vestibule with
one down ward sweep from the clitoris to the vaginal
os.
• The seventh sponge (optional in some) sweeps the area from the
fourchette down to the anus.
✔ The direction of stroking the areas to be prepared are:
✔ Anterior to posterior (symphysis pubis to anal region).
✔ Center to periphery (from inner inguinal region to midthigh).
• Finally wash the vulvar, perineal and thigh areas with sterile
water.
