1. Status

Public

2. Purpose

To provide a tracking system in the maintenance process of computer


laboratories.

3. Scope of Application

All CSS technicians, laboratory coordinator

4. Policy Content

All technical support staff assigned to a laboratory is expected to


maintain said laboratory as stipulated in the College by-laws or as
instructed by the HoS-CSS/HoC-ETC.
All technical support staff assigned to a laboratory should report any
damaged equipment, fixtures or furniture to the HoS-CSS accompanied
by a written report.
All technical support staff assigned to a laboratory should accomplish
CSS-F10 Computer Laboratory Maintenance/Additional Tasks Report on a
monthly basis.
The laboratory coordinator should accomplish CSS-F14 Maintenance
Summary Report for Computer Laboratories on a monthly basis to HoS-
CSS/HoC-ETC.

5. Procedures

a. The technical support staff accomplishes the form CSS-F10-Computer


Laboratory Maintenance Form for any maintenance conducted and keeps a
monthly record of it. These are, but not limited to, machine/equipment
check-ups, hardware/software installation, formatting of hard drives, or
any other computer/educational equipment related maintenance job. Any
additional tasks performed by the technician outside his/her assigned
laboratory should be stated in the form as well.
b. The technician should write the details such as date of maintenance,
status of: computer/AC/projector, etc., software installed, type of
maintenance performed, requests made, and remarks (if any).

c. The technician keeps the form for future reference and as evidence for the
maintenance conducted.
d. The technical support staff submits a copy of CSS-F10 to the laboratory
coordinator within one(1) week after the end of every month for checking
and report filing purposes.
e. The laboratory coordinator gathers all report from laboratory technicians
and accomplishes form CSS-F14-Maintenance Summary Report for
Computer Laboratories on a monthly basis. The said form summarizes per
laboratory, e.g. Lab1, Lab 1, Multimedia Lab, Open Lab, etc., what is
written in form CSS-F10 submitted by laboratory technicians.
f. The laboratory coordinator submits the summary report to the HoS-CSS
and/or Internal Auditor for checking and for report filing purposes.
