CONTRACT OF SERVICES
KNOW ALL MEN BY THESE PRESENTS:
The contract made and entered by and between:
VILLEGAS ELEMENTARY SCHOOL with office address at Brgy. Villegas,

Pozorrubio, Pangasinan, herein represented by MYRNA T. LIGOT, Head Teacher III,
hereinafter referred to as the “FIRST PARTY”;
and
JOSELITO L. ALAMBRA, of legal age, Filipino and a resident of Brgy. Villegas,

Pozorrubio, Pangasinan, hereinafter referred to as “SECOND PARTY”;
~WITNESSETH~
1. That the FIRST PARTY is in need of the services of the SECOND PARTY who shall
perform the work not performed by the regular personnel of the FIRST PARTY;
2. That the SECOND PARTY has signified the intention to which the FIRST PARTY has
accepted to provide services needed by the latter;
3. That the SECOND PARTY possess the skills required to safeguard the ICT equipment
located at the FCCCI building and the school premises DURING NIGHT TIME as well
as to serve as immediate response party for emergency cases during day time as
pupils’, teachers or staff’s welfare is considered;
4. That the SECOND PARTY attests that he is not related within the third degree of
consanguinity or affinity to: a) the Hiring authority; b) Representatives of the FIRST
PARTY; c) that he has not been previously dismissed from the government service by
reason of administrative offense; and d) that he has not reached the compulsory age
of retirement of sixty-five (65);
5. That, in view hereof, the SECOND PARTY hereby is contracted as SECURITY
GUARD with a monthly honorarium of TWO THOUSAND FIVE HUNDRED PESOS
(P2,500.00) only for a period of six (6) months, that is starting from March 1, 2020 to
August 30, 2020 due to the scarce fund from the monthly MOOE of the SECOND
PARTY;
IN WITNESS HEREOF, both parties have hereunto set their hands this 27 th day of
February, 2020 at Pozorrubio, Pangasinan.

DEED OF DONATION
This DEED OF DONATION made and expected this 20th day of JANUARY 2020 at
VILLEGAS ELEMENTARY SCHOOL, Villegas, Pozorrubio, Pangasinan represented by
ENGR. MARINO COLCOL DULAY hereinafter collectively called the DONOR.

VILLEGAS ELEMENTARY SCHOOL represented in this act by its Head Teacher III,
MYRNA T. LIGOT, the DONEE.
WITNESSETH:
That for and in consideration of the love and concern that the DONOR have for the
present and future learners of their alma mater, Villegas Elementary School and in sincere
token of their desire to help in the education of the learners in the said school, the DONOR
have freely and voluntarily given the amount of EIGHTEEN THOUSAND PESOS
(Php18,000.00) for the three (3) spans of school perimeter fence.
