INSTRUCTIONS
Host Name: Enter the name of the host.
OS Type: Enter the name of the operating system.
Start/End: Enter the date the backup is to begin, and the date the backup is to stop, if any.
Frequency: Enter how often the backup is to be performed (Daily, Weekly, Monthly, Quarterly, or Annually).
Retention (Days) Enter the number of days the backup is to be retained.
E-Mail Suite or Application: Enter the name of the e-mail suite (such as Outlook or Lotus Notes) or application that is to be backed up.
Database Type (db) Version If a database is to be backed up, enter the type, version, and approximate size.
& Approx. Size:
Directories: Enter the name of the directory that is to be backed up. If there are multiple directories, use one line for each directory
name. If the entire system is to be backed up, simply enter “Full System”. (Also, see Exclusion below.)
Exclusion:  If there is an exclusion that is related to the entry made in the Directories column, enter the name(s) of the file(s)
here. For instance, if “Full System” is entered under Directories, and there are exclusions, enter the directory
name(s) below “Full System” in the Directory column, and then enter the file name(s) in the Exclusion column.
 If ALL of the files located in the directory that is named in the Directory column are to be excluded, simply place an
“X” beside “ALL” in the Exclusion column.
Dependencies: If there is a dependency that must be addressed prior to the requested backup, explain it here.
