    documents:Government Issued ID of Engr. Dennis M. Francisco, Site Development Plan, Photos of
    Project Area taken within 15 days from filing, Notarized Accountability Statement of Engr. Dennis M.
    Francisco, & Proof of Payment within (15) days from the issuance of this order. Otherwise, the
    application will automatically be removed from the system and the project information has to be re-
    encoded to generate a new order of payment.
