C. EVALUATE ANCHOR & TEMPLATE CONDITION AFTER CONCRETE PLACEMENT (PROCEDURE ITEM ... CPI REQUIREMENT)
Setting and securement templates used to secure anchor bolts during
concrete pouring used sturdy, thick coated plywood templates
(Structural pedestals) & steel templates (Equip, Vessels)
A) Template restricted anchor movement (Hole clearance minimal).
BEST
B) Securement (template to anchor) was per nuts & washers.
PRACTICE
C) Anchor ends were fully taped (Concrete spatter prevention).
C1 Cost Savings
TEMPLATE CONDITION/DIMENSIONS CHECKED. OK TO REUSE.
& Good
FINAL RESULT = ANCHOR & ALL THREADS ARE UNDAMAGED.
Workmanship
1. Remove tape used that covered anchor threads during concrete.
2. Remove any concrete spatter that is stuck to threads & anchors.
3. Spin a nut to check anchors & suspect areas (damaged thds)
4. Perform req'd thread & galvanized coating repairs. See 3 Notes.
REMARKS:
' Support foundation is released for structural or equipment installation
' Support foundation is not released for structural or equipment installation.

NOTES: Use the Attachment for any Galvanized repairs

1. PERFORM ANY REQUIRED GALVANIZED COATING REPAIRS BEFORE INSTALLATION PER SAIC-M-2004 ATTACHMENT PROCEDURE.
2. THREAD DAMAGE (MASHED THREADS, ETC) IS CAREFULLY REPAIRED (MINIMIZE GALVANIZE REMOVAL) USING A KNIFE EDGE FILE.
3. NUTS SPIN FREELY & PROPERLY AFTER REPAIRS. PERFORM "COLD" COATING REPAIR OF REPAIRED THREADS. CURE. NUT TEST.
