Respectfully returned to the In-Charge, Office of the CENR Officer, the report of ocular
inspection and investigation conducted by the undersigned to the premises of the subject
parcel of land, the following facts, findings and recommendation are ascertained to wit:
That the parcel of land subject herein identified as Lot No. 2185, CAD 562-D located at
Linmansangan, Binalonan, Pangasinan containing an area of Six Thousand Five Hundred
Eighty-Three (6,583) square meters and same was surveyed for No Listed Claimant. Copy
of BL Form V-37 is hereto attached marked as Annex “A”. That on April 19, 2022 an
Affidavit of wrong claimant was executed as the survey party who undertook the survey of
Brgy. Linmansangan, Binalonan, Pangasinan made a mistake in recording No Listed
Claimant as wherein truth and in-fact the real owner and possessor is Rodrigo A. Soriano
and was sold to Sps. Hwaren R. Malicdem and Fatima G. Malicdem, copy of affidavit is
hereto attached marked as Annex “B”.
That Lot No. 2185, CAD 562-D, was originally in actual continuous, open, notorious and
exclusive possession and occupation by Rodrigo A. Soriano. This can be supported by Tax
Declaration No. 11-0010-02949, which copy is hereto attached marked as Annex “C”.
That on November 21, 2021, Rodrigo A. Soriano sold a portion of land to Sps. Hwaren R.
Malicdem and Fatima G. Malicdem duly notarized by Atty. Antonio Tiong. Copy of Deed of
Absolute Sale is hereto attached marked as Annex “D”. In support of the ownership of Sps.
Hwaren R. Malicdem and Fatima G. Malicdem attached herewith is the Tax Declaration No.
11-0010-20976 in the name of the latter and marked as Annex “E”
That due to the desire of Sps. Hwaren R. Malicdem and Rodrigo A. Soriano to subdivide
the said lot in three (3) lots and to avail of the “Handog Titulo Program” of the government
and to established their actual and corresponding shares and boundaries, they engaged
the services of Engr. Reinard B. Mariano to execute the subdivision survey Engr. Reinard
B. Mariano, private surveyor executed the necessary Subdivision survey in the following
manner, to wit:
That after executing the necessary subdivision survey, A Subdivision Plan was prepared
and same was confirmed by Sps. Hwaren R. Malicdem and Rodrigo A. Soriano. Blue print
copy of the Subdivision Survey is hereto attached marked as Annex “G”.
That the parcel of land subject herein per DENR, CENRO-Urdaneta City Certification dated
November 12, 2021, certifying that the subject parcel of land is not covered by any public
