TRAINING SESSION EVALUATION FORM

INSTRUCTIONS:
This post-training evaluation instrument is needed to measure how
satisfactory your trainer has done his job during the whole duration of your
training. Please give your honest rating by checking on the corresponding
cell of your response. Your answer will be treated with utmost
confidentiality.
