This form is required with your purchase of a Class B AIS and must be completed before your
order can be processed. Please e-mail this document completely filled out to:
[email protected] along with a copy of your MMSI registration.

For vessels that sail internationally:


Please e-mail [email protected] a copy of FCC Ships Station License with MMSI and Call Sign.

For vessels on domestic voyages only:


Please e-mail [email protected] a copy of MMSI Registration From Sea Tow, US Power
Squadron, Boat US or Other.
