Background

 Republic Act (RA) No. 10912 of the CPD Act

 Effectivity - August 16, 2016

 Implementation – March 15, 2017 upon effectivity of the


PRC Resolution No. 1032, s. 2017 or the Implementing
Rules and Regulations (IRR) of RA No. 10912.
Continuing Professional Development Accreditation
System (CPDAS)

 Adopted online platform by the Commission since


October 1, 2020 for the applications for accreditation as a
CPD Provider, of CPD programs, and of Other Activities for
Accreditation (self-directed learnings activities).

Requirements
1. Registration with the CPDAS through https://cpdas.prc.gov.ph
2. List of officers with valid Professional Identification Card or valid Government/Company-issued ID of each
officer from the applicant
3. Three-Year Annual Plan of Proposed CPD Programs
4. Accomplished CPDD-16 Form – Instructional Design for the first proposed program
5. Appointment paper from the managing partner authorizing the subject partner to manage CPD programs;
or Board Resolution/Secretary's Certificate of a Corporation authorizing an officer to manage CPD Programs
6. Accomplished CPDD-06 Form – Affidavit of Undertaking
7. Notarized copy of JVA or MOU or MOA, if applicable
8. For Individual/Sole Proprietor - DTI Certificate of Registration and Profile which includes Mission, Vision, and
Core Values
9. For Firm/Partnership/Corporation - SEC Certificate of Registration and Articles of Incorporation or
Partnership and their respective By-laws and Profile which includes Mission, Vision, Core Values SEC
10. BIR Certificate of Registration and Tax Clearance BIR
11. Mayor's or Business Permit

Note: Government offices as CPD providers are exempted from paying any processing fee,
except when programs to be offered will have registration fee, they must pay the processing fee
as stated above.

Once the documents are complete Processor will send you a message for

• Click Select PROGRAMS in the menu bar and select “Click Here to Apply a Program” as shown below
Requirements
1. Username and Password to access CPDAS Website.
2. Accomplished form CPDD-16 – Instructional Design or using the prescribed template by the Board.
3. Program of activities showing time/duration of topics/workshop and from the applicant Resource Persons
assigned for each topic.
4. Evaluation method or tool that measures the learning gained by the participants specific and appropriate to
course objectives set (pre-test and post-test questionnaire).
5. Resume of the resource person/s (accomplished form CPDD-17) including the e-/signature and valid
professional identification card/receipt of renewal of each resource person (if licensed professional) or valid
government/company issued ID of each resource person.
6. Breakdown of expenses (using the prescribed template)
7. Valid Special Temporary Permit (STP) if the resource person is a foreigner and if engagement is more than
three (3) days or there is physical contact with patients in the case of medical and allied professions, if
required.
8. For online or video-on-demand learning: (1) declaration of minimum technical requirements; (2) data privacy
notice (composed by the provider)
9. Consolidate and save as one (pdf) file.

Note: Any request to be made is subject to the approval of the concerned Council and this can only be used once per
Accredited Program

After the conduct of the accredited Program, attendance of the participants is required to be
encoded/uploaded. Click ATTENDANCE ENCODING button as shown below:

Note: Provider can choose between uploading the file (with proper format) or manually encoding the attendance of
participants

Shall be accomplished 30 days AFTER the conduct of the Program and the hardcopy shall be
submitted to the nearest PRC office as also required.

 Check our website cpdas.prc.gov.ph > Create an account and apply as a Professional

For your Masteral/PhD:


 Upload Certified True Copy of TOR; and
 Narrative Report on what has been learned

For Non-accredited Seminars and Trainings:


 Upload certificates and program of activities or narrative report on what has been learned
per training, that were conducted from 2019 up to present and related to your profession.

NOTE: Payment: 500 php per application. Your application will be forwarded to CPD Council for
verification. Processing time will take 15 days. Once your application has been approved it will
automatically reflect on your account and you may use it on your next renewal.
