    AFFIDAVIT
             I, RICO B. GERON, of legal age, Filipino, married and a resident of Madonna Homes, Alangilan, Batangas City,
    after having been duly sworn to in accordance with law, depose and state:

    1. That I am the authorized representative of SOROSORO IBABA DEVELOPMENT COOPERATIVE (SIDC), a


    duly registered cooperative under the Cooperative Development Authority and with business address at
    Sorosoro Ibaba, Batangas City;

    2. That we are applying for reclassification of our lands located at Brgy. Sampaguita, Naujan Oriental Mindoro
    with a total land area of 6.3533 hectares covered by TCT No. 064-2017000907(2.3334 ha), TCT No. 064-
    2017000877(2.335 ha) and TCT No. 064-2017000878(1.6864 ha);

    3. That the said reclassification of land is from agricultural to agri-industrial land with specific proposed use
    and services such as Gasoline Station, Coopmart (One Stop Shop), Coopmart Storage Room, Hardware
    Store, Hardware Storage Room, Hardware Stockyard, Admin Building, Palay Storage Building, Ricemill
    Warehouse with Canopy, Mechanical Dryer and Solar Dryer including parking area and driveway;

    4. That this affidavit is being executed in compliance with Department of Agriculture(DA) Administrative Order
    No. 01, Series of 2017 and as an attachment to our sworn declaration of application for land use
    reclassification under Doc No. 314; Page No. 63; Book No. XXII; Series of 2017 of Notary Public Atty. Jaero
    Garcia dated December 14, 2017; and

    5. That this affidavit is executed to attest to the truth of the foregoing facts and for whatever legal intents and
    purposes that it may legally serve.
