Dear Sir/Madam:
Undersigned is respectfully requesting for the lifting/continue processing of abandonment in the e2m system of the shipment consigned to ______________________ covered by BL No _____________which arrived at the MICP Port on April 18, 2021 on board Wisdom Grace 2111E with registry no SIC0063-21 and in support under oath, thereof hereby depose and state that:
1. The shipment was tagged as “abandoned” in the e2m system for failure to file the good declaration within the prescribed period due to late of shipping documents;
2. Thus, the request for lifting/continue processing of abandonment falls under Section 3 of CMO No. 24- 2015;
3. The request for lifting/continue processing of abandonment is not intended to violate or circumvent any laws, rules and regulation;
4. The consignee undertakes to take full responsibility for whatever consequences or liability that may result from approval of the request and release of shipment;
5. Attached are the following documents in support of subject request, to wit:
