We are pleased to inform you that based on the results of your contractual evaluation; we would like to
confirm your appointment as REGULAR employee under the following conditions:

1. Effective Date: You shall be a regular employee of this company effective on August 26, 2020.

2. Compensation & Benefits: That your basic Rate is Php 373.00 / day + overtime pay in
accordance to Philippine regulations.

3. Service Incentive Leave: Under Article 95 of Labor Code of the Philippines, every employee who
has rendered at least one (1) year of service is entitled to Service Incentive Leave (SIL) of five (5)
days with pay. The phrase “one year of service” of the employee means service within twelve (12)
months, whether continuous or broken, reckoned from the date the employee started working. The
period includes authorized absences, un-worked weekly rest days, and paid regular. The service
incentive leave may be used for sick and vacation leave purposes. The unused service incentive
leave is commutable to its money equivalent at the end of the year after it was reckoned.

4. Company Rules and Regulations: You are required to comply with the all existing rules,
regulations and policies of the Company as well as those which may hereafter be issued,
including but not limited to those governing orders and discipline, honesty, safety and security,
work assignments and standard operating procedures, use of Company properties and access
to matters of confidentiality, and such other rules deemed necessary in the conduct of our
business; On signing this contract, the EMPLOYEE acknowledges his duty and responsibility to
be aware of the rules & regulations of the employer regarding his employment and to fully comply
with this in good faith.

5. CONFIDENTIALITY: You agree that all records and documents of the Company and all
information pertaining to its business and/or its affairs and that of its customers are
absolutely confidential and unauthorized disclosure or reproduction of the same will not be
made by you at any time during or after your employment. You agree that any breach of
confidentiality will constitute sufficient ground for immediate termination of your
employment for cause and/or civil and criminal liability;

6. ASSIGNMENT OF TASK: You agree to be assigned or re-assigned to perform such other


tasks within the organization of the Company for such periods as may be determined by the
Company and whenever the service requires such assignments;
7. TERMINATION OF EMPLOYMENT: Aside from the just and authorized causes for the
termination of employment enumerated in Article 282 to 284 of the Labor Code, you might be
terminated also if we found out that you intentionally or unintentionally violate the Company
rules, policies and regulations.
In the event that you wish to terminate your contract of Employment for any reason, you must give
thirty (30) days written notice to Employer prior to the effective date of termination. Upon
termination of this employment, you shall promptly account for return of the following: Company
ID, CRR Employees handbook, Company property which may have been assigned or entrusted to
your care or custody.

In addition Last Pay will be given 30 days after the exit clearance for the finished contract and
resigned employee and 60 days for AWOL and Abandon employee.

If you agree with the above terms and conditions, please indicate your conformity by signing on
