    This is to endorse above named patient to undergo Pre-employment Examination, and to bill back EMAPTA
    PHILIPPINES INC. (company) of said test, to include:

    ( ) Medical History and Physical Examination


    ( ) Complete Blood Count (CBC)
    ( ) Urinalysis
    ( ) Fecalysis
    ( ) Chest X-ray
    ( ) Drug test (Shabu & Marijuana)

    Special Instructions to Examinee/Applicants:


    • Please present the endorsement letter from your company, together with a valid ID with picture upon
    availment.
    • Please come to the clinic in casual attire. Please refrain from coming in for your exam warring
    sando/slippers.
    • Urine specimen containers will be given at the clinic. Please collect midstream urine sample during day of
    examination and submit immediately to the laboratory. For female applicant with menstruation, please
    inform the laboratory and you will be asked to return 5 days after your menstruation for completion.
    • Please provide a thumb-sized stool sample to be placed in a clean container and must be submitted to the
    laboratory within 1-2 hours from time of collection.
    • No need for fasting preparation.
    • Please come atleast 15 minutes prior to your appointment schedule. If you have an emergency and need
    to re-schedule, please call us at least a day or an hour prior.
    • Wearing of SURGICAL MASK and FACE SHIELD is a must inside the clinic.
    • All patients will undergo screening and will fill up necessary forms: Checklist, CIF and consent. As a
    precautionary measure, bring your own pen to lessen risk of spreading the virus.
    • If patient experiencing any signs and symptoms of Covid within this last 2 weeks until the date of their
    appointment, kindly call for the clinic for rescheduling.
