1. Who is responsible for the policy that is being assessed? 2. Describe the main aims, objectives and purpose of the policy Tim Tebbs Borough Director Hammersmith and Fulham The purpose of this project is to relocate a) services and b) staff offices currently sited at Milson Road Health Centre. This will release the surplus estate to fund developments in the north of the borough. The services affected are: Podiatry, Leg Ulcer Clinics, Paediatric Audiology, Child Health and Child Development Clinics, Family Planning Service and a Midwifery Clinic. The following teams will need office relocation: Health Visitors, School Nurses, District Nurses and the Hospital at Home Team.

100611 Diabetes Services are a significant proportion of the services delivered at Milson Road and these are due to move to new premises on the Charing Cross Hospital site. There has been extensive public and patient engagement on the development of the diabetes pathway and therefore this is out of scope for the consultation exercise being undertaken and for this equalities impact assessment. Diabetes Services are due to move to the new sites later this summer (2011). 3. Are there associated objectives of the policy? If so, please explain. Examples include statutory requirements, sector initiatives, etc.

In planning relocation of services this project is informed by the PCTs long term objectives as set out within the Strategic Plan 2009-2014 with selected objectives stated below. We must maximise opportunities to improve NHS buildings to make productive use of all the space available and provide the environment patients and staff deserve. We must also make the most of opportunities through integration with the council to deliver more health services from other locations such as childrens centres and schools. Integration of frontline childrens services with community based provision for universal, services, vulnerable children and those with complex needs. Strengthening community services with school health model and health visitors working in Safeguarding teams. The integration of frontline community services with the council will support improved productivity with streamlined processes, enhanced skill mix within teams and less duplication.

Patients living within Hammersmith and Fulham. The public in and around Milson Road and patients currently receiving services at Milson Road.

100611 6. How has the policy been explained to those who would be directly or indirectly affected by it? 7. What outcome(s) are meant to be achieved from this policy? An engagement plan has been developed for the consultation period. This policy will make more services more accessible to the population in the centre and north of Hammersmith and Fulham where there are greater health inequalities. Release of the Milson Road Health Centre estate will release funding to contribute towards the development of services in the more deprived areas in the north of the Borough. Improved integration of services with GP Practices and realisation of the diabetes pathway.

Here are some examples: Positive impact: the relocation of services will build on the range of services currently offered at other locations in the North of the borough providing patients with improved access and choice of service delivery points. The relocation and co-location of health visitors within children centres with Speech and Language therapists and school nurses will improve integrated working. Negative impact: patients living in the close vicinity of Milson Road Health Centre receiving services at this location may need to travel further to receive these services. This may impact on the patient experience and accessing services. Neutral impact: relocation of staff offices is not considered to have a positive or negative impact on patient care. The re-provision of service at Milson Road gives the opportunity to provide services in other part of the Borough with greater choice offered in the north of the Borough where there are higher health inequalities.

Families with children

Moving Family planning services to Richford Gate will give greater access to this service for people living in the north of the Borough. Maintaining child health and development checks at The Masbro Centre (next door to Milson Road Health Centre) will maintain walk in access point for families and children locally.

There is a greater diversity of communities within the North of the Borough and the re-location of services will bring more access points to these communities.

Services will be relocated to sites that comply with the DDA. Patients currently supported to access Milson Road will be able to use existing support (Taxi Card ,Dial-aride) to access services elsewhere. Patients using mobility scooters may need to travel further to access services if they reside near Milson Road. All patients with identified mobility issues will be supported by the services to ensure this is addressed to the patients satisfaction. N/A

-Child development checks and health clinics will be relocated next door to the current delivery point. -The family planning services will be located at Richford Gate which is closer to main transport routes in the north of the borough.

Patients aged 50+ may benefit from the relocation of services but may also be impacted negatively if their mobility affects their ability to access services further away from their homes. This will need to be addressed by each service to ensure that mobility issues are identified and patients are supported to access services.

Step 4 Promoting equality


11. Please give a brief description of how this policy promotes equality. The scheme promotes equality in terms of health outcomes by improving the number of access points to people living in the north of the borough. An audit of patients by each service provided at Milson Road will establish patient profiles and inform patient access and monitor profiles of patients accessing re-located services. NO
