Lesson Overview

 Schools Under Department of Education (DepEd) and any private schools adopts
various school forms for various purposes. Teachers are to prepare and submit these
forms to concerned offices and School Head as part of their job functions.
 DepEd forms include the following but not limited to:

 School Form 1 (SF 1) School Register


 School Form 2 (SF2) Daily Attendance Report of Learners
 School Form 3 (SF3) Books Issued and Returned
 School Form 4 (SF4) Monthly Learner's Movement and Attendance
 School Form 5 (SF 5) Report on Promotion & Level of Proficiency
