Activities were being conducted without the presence of supervisors and appropriate fall protection measures like safety harnesses were found without proper inspection markings. 3. Electrical installations were found to have defects like improper cable insulation and joining. The site was instructed to stop major non-compliant activities, take corrective action on medium non-compliances, and immediately correct minor issues.
