    AFFIDAVIT OF LOSS
    I, FELIMON P. VILLAR, of legal age, Filipino, married and a resident of
    Adtuyon, Pangantucan, Bukidnon, after having been duly sworn to in accordance
    with law do hereby depose and say:

    1. That sometime today, while driving my motorcycle and upon arrival at


    Helen Branzuela store, located at Poblacion, Pangantucan, Bukidnon, I
    noticed that my wallet which was placed in my pocket was lost.

    2. That the contents of my said wallet are the following, to wit;

    a. Professional Driver’s License bearing No. ___________;


    b. Photocopy of Certificate of Registration (CR) and Official Receipt
    (OR) registered to Antonio Garces;
    c. Covid-19 vaccination card; and
    d. Cash amounting to Php1,300.00

    3. I honestly believe that the said wallet fell somewhere which I did not
    notice.

    4. That diligent efforts were exerted to locate the above-mentioned wallet as


    well as the contents thereof but the same proved to no avail;

    5. That the mentioned Professional Driver’s License was not confiscated by


    any authority for violation/s of law.

    6. That I am executing this affidavit to apprise the authorities of the


    surrounding circumstances and to facilitate my request for its re-issuance;

    7. Further, I am executing this affidavit to attest to the veracity of the


    foregoing facts and for whatever worth this may serve best.

    IN WITNESS WHEREOF, I have hereto affixed my hand this 31 st day of


    December, 2021 in Pangantucan, Bukidnon.
