HAZOP Failure Checklist

Problem Strategy
Poor facility Preparation, dedicated facility, notice on door, sufficient
room for participants, projector/ display checked before
meeting. Sufficient space for both minutes and drawings.
Culture (acceptance of criticism) Company HAZOP policy – document the expected style of
the review.
