DISSCUSION

In this chapter we will discuss the patient’s baseline data, health history, laboratory

results and the possible cause of the decrease of her amniotic fluid which the main cause of the

death of her fetus inside her womb.

According to Krissi Danielsson (January 2020) that fetal death occurs when a fetus dies

spontaneously inside the uterus at any point during pregnancy. Fetal deaths that occur later in

pregnancy (at 20 weeks or more, or at 28 weeks or more, for example) are also known as

stillbirths. In the United States, state laws compel fetal death reporting, while federal law

mandates the collecting and dissemination of fetal mortality statistics on a nationwide scale.

Most states record fetal fatalities with a gestational age of 20 weeks or more and/or a birth

weight of 350 grams or higher. Only a few states, however, report fetal fatalities throughout all

stages of pregnancy. Danielsson K. (January 2020) added that Fetal mortality happens in around

one out of every 100 pregnancies in the United States, according to the Centers for Disease

Control and Prevention. The likelihood of an early stillbirth (between 20 and 27 weeks) is just

slightly higher than that of a late loss (28 weeks or later). According to the Centers for Disease

Control and Prevention (CDC), there are around 24,000 stillborn births in the United States. The

gestational age and weight of the fetus are used to determine fetal demise in different parts of the

globe. The threshold might vary from at least 16 weeks to at least 26 weeks, with a weight of at

least 400 grams to at least 500 grams, depending on where you live.

Based in the data given, all her vital signs are normal so basically, the decrease of her

amniotic fluid is the only possible cause of the death of the fetus in her utero. Oligohydramnios

(Low of amniotic fluid) targets the issue of Placental Dysfunction of the mother. The website

parentinghealthybabies.com (April 2018) proposes that the mother may have insufficient
amniotic fluid if the placenta is unable to give enough nutrition and blood to the fetus. High

blood pressure, diabetes, lupus, and pre-eclampsia can all cause placental problems. Placental

issues might occur if a pregnancy lasts longer than 42 weeks. Windowofworld.com (November

2019) stated that a malfunctioning placenta is one of the causes of intrauterine fetal death

(IUFD). The distribution of critical nutrients needed by the fetus throughout the womb, such as

blood flow and oxygen, is restricted and decreased when the placenta is disrupted. As a result,

fetal development can be hampered (intrauterine growth restrictions/IUGR) and intrauterine fetal

mortality can occur. The mother also has a family history of Diarrhea, Hypertension, and

Anemia which not the real cause of the death of the infant inside her utero.

Long-term oligohydramnios therapies have not shown to be very successful. There are a

few short-term therapeutic alternatives available. They can be done under particular

circumstances. If you're at least 36 weeks pregnant, delivery may be the safest option. To avert

problems, the doctor may recommend a cesarean section. Dehydration can sometimes be the

cause of decreased amniotic fluids. Oral and intravenous fluid treatment has been found to raise

amniotic fluid levels in such instances. Oligohydramnios can be a difficult condition to deal with

during labor. An intrauterine catheter can be used to provide saline. This extra fluid helps to

alleviate pressure on the umbilical cord during labor, increasing the odds of a normal birth.

Amnio-infusion is the name for this procedure. This technique can also be utilized to improve

ultrasound pictures in moms who have low quantities of amniotic fluid. Amniocentesis is a

procedure that involves injecting fluid into the amniotic sac before to birth. The disease improves

for a short time; however, the results only endure approximately a week, that is according to the

website parentinghealthybabies.com (April 2018).
