PARTICIPANT DECLARATION
I HEREBY CONFIRM THAT THE INFORMATION WRITTEN IN THIS FORM ARE ACCURATE AND COMPLETE TO THE BEST OF MY KNOWLEDGE. I UNDERSTAND THAT ANY
FALSIFICATION OF DETAILS WRITTEN IN THIS FORM CAN LEAD TO THE CANCELLATION OF MY APPLICATION TO THE PROGRAM
a participant of the Canada Study and Work Program (referred to as “Program”),
and facilitated by First Place, Inc. (referred to as “FPI”),
together with my Parents and/or Legal Guardian as follows:
hereby affirm that my parents and/or legal guardians and I (referred to as “We”)
have carefully read and fully understood, and hereby agree and bind ourselves to abide by and comply with all the terms and conditions set forth below, which shall
form part of my agreement with all concerned parties in this program.
I. THE NATURE OFTHE PROGRAM
a. My parents and/or legal guardians and I understand that the Canada Study and Work Program refers to the academic or vocational education a student receives
while studying in Canada. Once a Canadian Designated Learning Institute, also known as DLI, accepts a student’s admissions application, a designated school
official will send me, the prospective student a “Letter of Acceptance”. The letter must be issued by the Canadian institution (e.g. school district, University, etc.)
on official letterhead, show the exact amount of tuition fees I am required to pay, the anticipated starting and finishing dates and the date by which I need to
register. Once I have I acceptance letter, I can complete and submit an application for a study permit. Completing a program of study at a DLI is the primary
purpose for all students studying in the Canada. Completion of such a program is usually marked by receiving a diploma, degree or other type of certification.
b. In order to study in Quebec, students must obtain a Certificat d'acceptation du Québec (CAQ) issued by the Ministère de l'Immigration, Diversité et Inclusion
(MIDI). The educational institution should provide the student with all the details related to applying for Quebec.
c. My participation is voluntary and not part of any school requirements.
d. We fully understand the requirements and qualifications to join the Program. Eligible students may enroll in several different types of schools that offer certified
programs of study; these include kindergarten through grade 12 schools, community colleges and undergraduate and graduate schools. As a prospective student
interested in studying in the Canada, we understand that the terms “college” and “university” may be used interchangeably. We also understand that both refer
to schools that offer post‐secondary (i.e., after high school) educational instruction to students. However, a college usually refers to a single institution of higher
education. A university usually is a system with many post‐secondary schools that may offer both undergraduate and graduate level degrees.
e. We understand that FPI is a career counseling organization and not a Canadian Immigration consulting company. The role of FPI is to accept my application, conduct
orientations and provide information about the Canadian study program, and its partner DLI’s offerings, conduct assessment, interviews, tests, and background
checks of the applicants and their families and assist me throughout my application processes. Therefore, it is the sole discretion of FPI to endorse my application
to the DLI based on their internal guidelines and the requirements of the DLI. Since FPI is not a party to my agreements with these Canadian entities, FPI cannot be
held liable for any failure of the DLI to comply with their respective contracts and/ or agreements.
f. We fully support the mission of FPI to build careers and develop the character of its applicants by instilling personal initiative, self‐discipline, responsibility and
independence. My parents and/or legal guardians and I agree that all documents must be prepared and organized by me as the applicant and submitted on time.
All communications and personal follow up to monitor and clarify application status must be made only by me as the applicant. Parents and/or legal guardian’s
inquiries will only be entertained through email and not by phone or text. If needed, FPI shall invite all parties concerned to a meeting to fully clarify application
related issues. Failure to comply with any one of the above conditions shall be subject to penalties and/or suspension of the application process until all requirements
are complied with.
g. We fully understand that the standard form of communications of FPI with Students/Applicants is through official E‐MAILS. We understand that only
communications/statement sent through email will be deemed official and binding.
h. We fully understand and accept that while it may be possible to work for up to 20 hours weekly during my program, per IRCC guidelines, First Place Inc. does not
guarantee any work placements or work hours during my program. Furthermore, we fully understand and accept that First Place Inc. is not an immigration
counseling service and does not guarantee nor offer post‐graduation placements, permanent residence, citizenship, or any other immigration‐related assistance.
II. APPLICANTS, PARENTS and/or LEGAL GUARDIANS OBLIGATIONS and UNDERTAKINGS BEFORE the PROGRAM
8. My parents and/or legal guardians and I agree to attend all the orientation and program related briefing sessions in the Philippines, online or face to face, whichever
is applicable. We shall not hold FPI liable for any negative consequences that may result due to our lack of understanding of the Program requirements, rules and
regulations.
9. We here by guarantee the authenticity and accuracy of all documents we will submit to FPI, the DLI, IRCC (Immigration, Refugees and Citizenship Canada) and
Canadian Embassy. Failure to fulfill this guarantee will result in the cancellation of my program.
10. We will make sure that we have been given adequate briefing and instructions by FPI regarding the Program. We will comply with all the requirements and strictly
follow all instructions given by FPI and the DLI from the time of my application until the end of my study program. Failure to comply with any of these requirements
