Dr. George K. Ramos filed an affidavit of loss to attest that he lost his diploma from Saint Louis University. AFFIDAVIT OF LOSS

I, DR. GEORGE K. RAMOS, of legal age, Filipino, married, a resident of


61 Ferguson Road, Guisad, Baguio City, after having been sworn to in
accordance with law, hereby depose and state that:

1. I graduated last March 1982 from Saint Louis University, Baguio City,
having finished the degree Bachelor of Science in Commerce, Major in
Accounting;
2. By reason of my graduation from the said school, I was issued my
diploma;
3. I used to keep my said diploma together with other documents in a
folder which I placed at my locked drawer in my previous office table at Room
S428 of the Diego Silang Building of the College of Accountancy and Commerce
(CAC), Saint Louis University;
4. Sometime during the last week of May 2000, the offices of the CAC
transferred to its present location at the 5 th floor of the Diego Silang Building.
During the actual transfer, the drawers of the office table were removed so as to
facilitate the transfer;
5. Unfortunately, by reason of the transfer, some office paraphernalia,
personal effects, and documents were misplaced and/or were missing. Efforts
where then exerted to locate these things which led to the recovery of some
items; but the others remained missing;
6. Among those items that remained missing was my folder which
contained the aforesaid diploma;
7. I exerted all efforts to look for the said diploma but despite my diligent
efforts, I could not locate it anymore, for which reason I sincerely believe that
said diploma is permanently lost;
8. I am executing this Affidavit to attest to the truth of all the foregoing
facts.

July, 2004, at Baguio City, Philippines.
